Recommendati<strong>on</strong> 5-18: Where feasible, services should be shifted to lower-cost caregivers.<br />

Across <strong>the</strong> spectrum <strong>of</strong> caregivers, full scope <strong>of</strong> practice needs to be exercised.<br />

There should be a net shift in resp<strong>on</strong>sibilities from physicians to nurses and o<strong>the</strong>rs in health<br />

teams, including physician assistants. This should be supported by changes to fee schedules;<br />

for example, by not paying physicians for interventi<strong>on</strong>s like vaccinati<strong>on</strong>s that could be d<strong>on</strong>e<br />

by nurses.<br />

There is a significant number <strong>of</strong> cases where <strong>the</strong>re are minimal risks <strong>of</strong> complicati<strong>on</strong>s and<br />

nurses or physician assistants can play a more independent role; however, <strong>the</strong>re is an equally<br />

significant number <strong>of</strong> cases where <strong>the</strong>re is a somewhat higher but not extreme risk <strong>of</strong><br />

complicati<strong>on</strong>s where nurses or physician assistants would require a doctor to be nearby to<br />

deal with any potential issues. Family Health Teams have optimized a model that could be<br />

transferable to o<strong>the</strong>r settings as nurses are <strong>of</strong>ten in close proximity to doctors.<br />

Recommendati<strong>on</strong> 5-19: A broader perspective should be applied to decisi<strong>on</strong>s that are made<br />

<strong>on</strong> <strong>the</strong> scope <strong>of</strong> practice <strong>of</strong> health pr<strong>of</strong>essi<strong>on</strong>als. Government should play a more active role in<br />

working with <strong>the</strong> pr<strong>of</strong>essi<strong>on</strong>al colleges to apply a system-wide approach ra<strong>the</strong>r than dealing<br />

with individual pr<strong>of</strong>essi<strong>on</strong>s in isolati<strong>on</strong>.<br />

Doctors and nurses may discuss am<strong>on</strong>g <strong>the</strong>mselves what activities <strong>the</strong>y should retain or give<br />

up, but <strong>the</strong>re is no mechanism to put <strong>the</strong>se internal dialogues toge<strong>the</strong>r and get some acti<strong>on</strong> <strong>on</strong><br />

changes that both agree would be beneficial.<br />

Recommendati<strong>on</strong> 5-20: Maximize opportunities to use nurse practiti<strong>on</strong>ers with <strong>the</strong> aim <strong>of</strong><br />

efficiency, while maintaining excellent care.<br />

Seven <strong>of</strong> <strong>the</strong> c<strong>on</strong>trolled acts authorized to physicians are also authorized to nurse<br />

practiti<strong>on</strong>ers, for example: periodic health examinati<strong>on</strong>s (“full physicals” for children, women<br />

and men); assessment and clinical management <strong>of</strong> acute episodic illness, for example,<br />

respiratory tract infecti<strong>on</strong>s; and m<strong>on</strong>itoring stable chr<strong>on</strong>ic health c<strong>on</strong>diti<strong>on</strong>s, such as<br />

hypertensi<strong>on</strong> and diabetes, etc.<br />

Recommendati<strong>on</strong> 5-21: Recognize <strong>the</strong> increased demand for nurses in <strong>the</strong> capacity <strong>of</strong><br />

nursing programs at colleges and universities and <strong>the</strong>ir ability to train more nurses.<br />

There are already severe labour shortages for nurses and various technicians, with some<br />

reports <strong>of</strong> a 20 to 30 per cent attriti<strong>on</strong> <strong>of</strong> nurses within <strong>the</strong> first year after nursing school. There<br />

is a desperate need to increase supply and improve retenti<strong>on</strong>. If this is not addressed quickly,<br />

demand-supply imbalance will blow up labour costs and compromise overall restraint.<br />
