<strong>Milk</strong> <strong>and</strong> dairy products <strong>in</strong> human nutrition<br />

us<strong>in</strong>g a pre- <strong>and</strong> post-<strong>in</strong>tervention design to look at programme outcomes related<br />

to <strong>in</strong>come <strong>and</strong> milk consumption. Only adequacy <strong>in</strong>ference is possible from this<br />

design. Some process-evaluation data were also collected to look at goat production.<br />

Us<strong>in</strong>g the Helen Keller International (HKI) food frequency questionnaire,<br />

they determ<strong>in</strong>ed that children <strong>in</strong> the 39 households surveyed <strong>in</strong> Gorogutu District<br />

consumed milk more frequently follow<strong>in</strong>g the <strong>in</strong>tervention. In Gursum District,<br />

FARM-Africa found that more than 85 percent of the <strong>in</strong>tervention households,<br />

especially young children, consumed the goat milk they produced.<br />

While there were several positive nutrition-related outcomes such as <strong>in</strong>creased<br />

milk consumption <strong>and</strong> other dietary <strong>in</strong>take improvements, the DGDP was not able<br />

to demonstrate effects on cl<strong>in</strong>ical signs of vitam<strong>in</strong> A deficiency. In December 1995,<br />

a new trial was developed to test whether add<strong>in</strong>g an <strong>in</strong>tegrated package of <strong>in</strong>terventions<br />

to DGDP, <strong>in</strong>clud<strong>in</strong>g more-concerted efforts to promote vitam<strong>in</strong>-A-rich foods<br />

such as goat milk, would better achieve the nutrition outcomes (Ayalew, Gebriel<br />

<strong>and</strong> Kassa, 1999). Formative research was conducted to develop nutrition messages,<br />

<strong>and</strong> an <strong>in</strong>tegrated package that <strong>in</strong>volved work<strong>in</strong>g through women’s groups <strong>and</strong><br />

deliver<strong>in</strong>g messages through community education, radio <strong>and</strong> television broadcasts,<br />

<strong>and</strong> the <strong>in</strong>tervention was implemented from December 1996 to August 1997. Aga<strong>in</strong>,<br />

a pre-<strong>in</strong>tervention (February–March 1996) <strong>and</strong> post-<strong>in</strong>tervention (January–March<br />

1998) design was used for evaluation. In addition to the limitations imposed by<br />

this adequacy-level design, 32 percent dropout rate <strong>and</strong> compromised <strong>in</strong>tervention<br />

<strong>in</strong>tegrity (33 percent ma<strong>in</strong>ta<strong>in</strong>ed the same pattern of goat ownership while others<br />

acquired new goats or lost the orig<strong>in</strong>al goats) posed serious problems for analyses.<br />

The results showed that diversity of children’s diet was greater <strong>in</strong> the post<strong>in</strong>tervention<br />

period than pre-<strong>in</strong>tervention (Ayalew, Gebriel <strong>and</strong> Kassa, 1999). In<br />

the goat-own<strong>in</strong>g households, all the milk produced was consumed. However,<br />

<strong>in</strong>vestigators found that goat ownership alone did not reduce risk of vitam<strong>in</strong>-A<br />

deficiency but that milk consumption <strong>and</strong> <strong>in</strong>come from agricultural sales were<br />

<strong>in</strong>versely related to vitam<strong>in</strong>-A deficiency. These results further re<strong>in</strong>force the need<br />

for more <strong>in</strong>tegrated, comprehensive <strong>in</strong>tervention packages.<br />

Participatory research carried out <strong>in</strong> Liben <strong>and</strong> Sh<strong>in</strong>ile, Ethiopia, prior to<br />

develop<strong>in</strong>g <strong>in</strong>terventions to promote milk consumption (Sadler <strong>and</strong> Catley, 2009)<br />

demonstrated that Somali pastoralists’ have a strong appreciation for milk (R<strong>and</strong>olph<br />

et al., 2007). <strong>Milk</strong> supplied two-thirds of energy <strong>and</strong> 100 percent of prote<strong>in</strong><br />

for young children around one year of age. Children’s milk consumption varied<br />

between seasons <strong>and</strong> fell dur<strong>in</strong>g droughts. The communities identified <strong>in</strong>terventions<br />

to improve animal health (fodder production, <strong>in</strong>creased water supply <strong>and</strong> veter<strong>in</strong>ary<br />

care) as a means to ensur<strong>in</strong>g adequate supply of milk for consumption (Sadler <strong>and</strong><br />

Catley, 2009).<br />

7.3.2 Asia <strong>and</strong> the Pacific<br />

The VAC system is a traditional farm<strong>in</strong>g approach <strong>in</strong> Viet Nam that <strong>in</strong>corporates<br />

environmental, ecological <strong>and</strong> nutritional pr<strong>in</strong>ciples. V st<strong>and</strong>s for vuon or garden<br />

(all l<strong>and</strong> cultivation activities), A for ao or pond (all aquaculture), <strong>and</strong> C for<br />

chuong or cattle shed (all animal husb<strong>and</strong>ry). S<strong>in</strong>ce 1989, there have been positive<br />

trends nationally <strong>in</strong> poverty reduction, agricultural production, socio-cultural,<br />

environmental, <strong>and</strong> health outcomes (Hop, 2003). This ecological study of secular
