12003 MARCOR OSH PROGRAM MANUALc. Lockout devices shall be substantial enough to preventremoval without use of excessive force such as bolt cutters.d. Lockout identification tags are used in conjunction withthe locking device when performing a lockout. An example of atag is provided in figure 12-4. The tag identifies the personapplying the lock. It shall be singularly identifiable <strong>and</strong>capable of withst<strong>and</strong>ing the environment to which exposed withoutbecoming deteriorated or illegible. The tag shall bear the name<strong>and</strong> shop/code of the authorized worker, authorized worker'stelephone number <strong>and</strong> date of lockout.2. Tagout devicesa. In situations where equipment must be worked on <strong>and</strong>cannot be locked out, equipment shall be tagged out. An exampleof a tag is provided in figure 12-4. Tag equipment under repairor maintenance that could jeopardize the <strong>safety</strong> of workers ordamage equipment to alert personnel to prevent energization ofthe tagged equipment. Equipment will not be operated, workedon, or removed when tagged out.b. The tag <strong>and</strong> its means of attachment shall be strongenough to prevent inadvertent or accidental removal. Attachmentdevices shall be non-reusable, attachable by h<strong>and</strong> (no toolsrequired), self-locking, <strong>and</strong> non-releasable with a minimumunlocking strength of 50 pounds.12004. WRITTEN LOCKOUT/TAGOUT PROCEDURES1. Specific written procedures shall establish the minimumrequirements for lockout or tagout of energy isolating devices.NOTE: Specific SOPs for control of hazardous energy sources mustbe developed at the shop level for each piece of equipmentor machinery before maintenance or servicing is performed.Machines <strong>and</strong> equipment shall be evaluated using NAVMC11403, Lockout/Tagout Checklist, at figure 12-2.12-8
