MARCOR OSH PROGRAM MANUAL 14002(2) Application of preservatives, paint, epoxies <strong>and</strong>similar operations, which may generate hazardous concentrationsof toxic or flammable vapors.(3) Cleaning operations, sludge removal <strong>and</strong> similaroperations, which may produce or cause release of hazardousconcentrations of toxic or flammable vapors.(4) Any other similar operations which possess thepotential for producing or releasing toxic, flammable, orasphyxiating atmospheres or materials.NOTE: The CSPM shall ensure that appropriate instruments areavailable to perform required atmospheric testing. Theseinstruments shall be suitable for the task as indicated bycertification from the National Institute for OccupationalSafety <strong>and</strong> Health (NIOSH), Mine Safety <strong>and</strong> HealthAdministration (MSHA), or other nationally recognizedtesting authority such as Underwriters Laboratory (UL) orFactory Mutual (FM). The CSPM shall ensure equipment isproperly used, maintained, <strong>and</strong> calibrated per themanufacturer's instructions. Functional or calibrationchecks shall be made before <strong>and</strong> after use <strong>and</strong> a record ofthese checks shall be maintained for 1 year.c. Other Hazard Evaluations. In addition to potentiallyhazardous atmospheres, other confined space hazards such as thepresence of piping, slippery surface, unstable ladders,engulfment potential, <strong>and</strong> energy sources shall also be evaluatedprior to entry into a PRCS.4. Issuance of Confined Space Entry Permits. If entry into aPRCS is required, the responsible line supervisor shall requesta permit (NAVMC 11405) from the CSPM. Request shall include adescription of the space, operation to be performed, <strong>and</strong> a listof the personnel that will enter the space. Based on results ofthe comprehensive evaluation discussed above, the CSPM (or aqualified assistant working under <strong>dir</strong>ection of CSPM) shall grantthe permit only if entry or work can be performed safely.Permits shall be valid for a period of time specified by theCSPM, but not to exceed one work shift or 8 hours. Confinedspace entry permits will be distributed as follows:14-7
