14002 MARCOR OSH PROGRAM MANUALi. NAVAIR 01-1A-35, Aircraft Fuel Cells <strong>and</strong>Internal/External Tanks, provides <strong>safety</strong> precautions on themaintenance <strong>and</strong> repair of aircraft fuel cells which involveentry into or work on confined spaces. The CSPM or ACSPM mustalso complete the formal training required by NAVAIR 01-1A-35 tooversee fuel cell entry.6. Program Evaluation. Comm<strong>and</strong>ers shall ensure that anevaluation of the confined space entry program is conducted atleast annually <strong>and</strong> immediately following any serious mishap ornear miss mishap incident.14003. REQUIREMENTS FOR CONFINED SPACE ENTRY AND WORK.Issuance of a permit for confined space entry <strong>and</strong> work startswith a thorough evaluation, by competent <strong>and</strong> trained personnel,of potential hazards that may be encountered <strong>and</strong> establishmentof necessary control measures. In addition to hazardevaluation, the following requirements <strong>and</strong> restrictions alsoapply:1. Authorized Entry Personnel. Only personnel authorized bythe responsible entry supervisor shall be allowed to enter apermit required confined space. The responsible entrysupervisor shall ensure personnel are aware of hazards likely tobe encountered <strong>and</strong> the appropriate <strong>safety</strong> measures initiated.The responsible entry supervisor shall also ensure personnel arefully trained <strong>and</strong> medically qualified for such activity.2. Attendants. Attendants are m<strong>and</strong>atory for all entries intopermitted spaces. Attendants shall be equipped with radios orother communications equipment to ensure prompt emergencyresponse. The following requirements apply to attendants:a. Attendants shall remain outside of the main opening ofthe space to monitor conditions inside <strong>and</strong> summon assistance inevent of any emergency. This practice must be strictly adheredto because a high number of fatalities in confined spaces aredue to untrained personnel attempting a rescue.b. Attendants shall be instructed regarding propernotification procedures in event of an emergency <strong>and</strong> thatattendants may only attempt non-entry rescues until trained14-10
