MARCOR OSH PROGRAM MANUAL 90069004. OSH DEFICIENCY NOTICES. A NAVMC 11400, OSH DeficiencyNotice (figure 7-1) of the unsafe or unhealthful workingcondition shall be posted at the sight of the deficiency in ahighly visible location, in the immediate vicinity of thehazardous condition. This notice shall not be removed until thecondition has been corrected.9005. RETENTION OF REPORTS. Copies of unsafe <strong>and</strong> unhealthfulworking condition reports <strong>and</strong> records of abatement actions takenshall be retained at installation or unit <strong>safety</strong> offices for atleast 5 years after the end of the calendar year in which finalaction is completed.9006. ANYMO<strong>US</strong>E REPORTS. NAVMC 11509, ANYMO<strong>US</strong>E reports are aninformal way to report <strong>safety</strong> concerns anonymously. The reportbox <strong>and</strong> blank reports shall be posted in a discreet location asto prevent identification of the individual. It is intended toencourage personnel to make voluntary reports of <strong>safety</strong> concernsthat they are not comfortable reporting through the establishedchain of comm<strong>and</strong> or reporting channels. The vast majority ofANYMO<strong>US</strong>E reports will be resolved at the local level. SeeFigure 9-3.1. The <strong>safety</strong> office shall investigate all NAVMC 11509 ANYMO<strong>US</strong>Ereports <strong>and</strong> inform the comm<strong>and</strong>ing officer within 30 days ofreceipt. Upon investigation <strong>and</strong> if conditions warrant, bringconcerns to the appropriate supervisor for corrective action.Alleged imminent danger situations shall be investigated within24 hours. If the report involves a health hazard, the <strong>safety</strong>office shall coordinate efforts with the local medical facilityfor investigation.2. A response to the NAVMC 11509 ANYMO<strong>US</strong>E report should beposted within 35 days of receipt informing the originator of thecorrective action or advising that no hazard exists.Notification of receipt shall be posted near the ANYMO<strong>US</strong>E box,on the <strong>safety</strong> bulletin board, in weekly newsletters, or <strong>safety</strong>council meeting minutes, verifying receipt of the report <strong>and</strong>corrective action taken. If response time cannot be met for anyreason, information should be posted advising of such.9-7
