9006 MARCOR OSH PROGRAM MANUAL3. Personnel submitting a NAVMC 11509 ANYMO<strong>US</strong>E report shallcomplete the report <strong>and</strong> place it in an ANYMO<strong>US</strong>E box or locationdesignated to collect the report. The <strong>safety</strong> office willretrieve the reports <strong>and</strong> provide follow up by posting results onan ANYMO<strong>US</strong>E board, <strong>safety</strong> bulletin board, weekly newsletter, orminutes of <strong>safety</strong> council meetings.4. If an individual is not satisfied with the results, theyshould appeal to higher headquarters, or mail or fax the NAVMC11509 ANYMO<strong>US</strong>E report to CMC (SD), (703) 695-3231 or DSN 225-3231 to request resolution. Anonymity will be preserved.5. ANYMO<strong>US</strong>E reports may be used to highlight conditions thatmay add “lessons learned” value to others. These reports may beused to produce articles for <strong>safety</strong> magazines (e.g., GroundWarrior), newsletters, or used as a topic of discussion at<strong>safety</strong> st<strong>and</strong>downs.6. Upon investigation, if design or manufacturing flaws arefound, a hazard report message (HAZREP) shall be initiated perOPNAVINST 5102.1D/MCO P5102.1B.7. Additional information on the ANYMO<strong>US</strong>E program may beobtained at http:/www.hqmc.usmc.mil/<strong>safety</strong>.nsf.8. NAVMC 11509 ANYMO<strong>US</strong>E reports shall be retained for a periodof 3 years.9-8
