9006 MARCOR OSH PROGRAM MANUAL3. Personnel submitting a NAVMC 11509 ANYMOUSE report shallcomplete the report and place it in an ANYMOUSE box or locationdesignated to collect the report. The safety office willretrieve the reports and provide follow up by posting results onan ANYMOUSE board, safety bulletin board, weekly newsletter, orminutes of safety council meetings.4. If an individual is not satisfied with the results, theyshould appeal to higher headquarters, or mail or fax the NAVMC11509 ANYMOUSE report to CMC (SD), (703) 695-3231 or DSN 225-3231 to request resolution. Anonymity will be preserved.5. ANYMOUSE reports may be used to highlight conditions thatmay add “lessons learned” value to others. These reports may beused to produce articles for safety magazines (e.g., GroundWarrior), newsletters, or used as a topic of discussion atsafety standdowns.6. Upon investigation, if design or manufacturing flaws arefound, a hazard report message (HAZREP) shall be initiated perOPNAVINST 5102.1D/MCO P5102.1B.7.
