The RAC defined by a matrix representsthe degree of risk associated with a hazard considering theelements of hazard severity and mishap probability. The RAC isderived as follows:1. Hazard Severity. An assessment of the worst potentialconsequence, defined by degree of occupational injury, illnessor property damage which is likely to occur as a result of thedeficiency. Roman numerals shall assign hazard severitycategories according to the following criteria:a. Category I. May cause death, permanent totaldisability, or loss of a facility/asset.b. Category II. May cause permanent partial disability,temporary total disability in excess of 90 days (severe injuryor severe occupational illness), or major property damage.c. Category III. May cause minor injury, minoroccupational illness, or minor property damage.d. Category IV. Presents minimal threat to safety, health,or property, but is still in violation of a standard.2. Mishap Probability. The probability that a hazard willresult in a mishap or loss, based on an assessment of suchfactors as location, exposure (cycles or hours of operation),affected populations, experience, or previously establishedstatistical information. Mishap probability shall be assignedan English alphabet symbol according to the following criteria:a. Subcategory A. Likely to occur immediately or withina short period of time.b. Subcategory B. Probably will occur in time.c. Subcategory C. May occur in time.d. Subcategory D. Unlikely to occur.7-15
