MARCOR OSH PROGRAM MANUAL 120052. Exceptions. Written procedures are not required when all ofthe following elements exist:a. The machine or equipment has no potential for stored,residual, or reaccumulation of energy after shutdown.b. The machine or equipment has a single energy source thatcan be readily identified <strong>and</strong> isolated.c. The isolation <strong>and</strong> locking out of that energy source willcompletely deenergize <strong>and</strong> deactivate the machine or equipment.d. The machine or equipment is isolated from the energysource <strong>and</strong> locked out during service or maintenance.e. A single device will achieve a locked out condition.f. The lockout device is under exclusive control of theauthorized worker performing service or maintenance.g. The service or maintenance does not create hazards forother personnel.h. In utilizing this exception, no accidents have occurredinvolving the unexpected activation or reenergization of themachine or equipment during service or maintenance.12005. PERFORMING LOCKOUT/TAGOUT PROCEDURE1. Preparation for Lockout/Tagout. The authorized worker shalllocate <strong>and</strong> identify all isolating devices to be certain whichswitch(es), valve(s), or other energy isolating devices apply tothe equipment to be locked or tagged out. More than onehazardous energy source or means of disconnect (electrical,mechanical, or others) may be involved.2. Lockout or Tagout System Procedurea. Notify all affected personnel <strong>and</strong> responsible supervisorthat a lockout or tagout procedure is going to be utilized. Theauthorized worker shall know the type <strong>and</strong> magnitude of energy12-9
