MARCOR OSH PROGRAM MANUAL 90037. After receipt of a report of unsafe or unhealthful workingcondition, a <strong>safety</strong> office written response will be provided tothe originator within 15 days either verifying the reportedcondition <strong>and</strong> providing corrective actions, or stating reviewconcluded <strong>and</strong> no hazard exists. If response time cannot be metfor any reason, an interim reply will be made. If anonymity hasbeen requested, a notice should be posted on the official or<strong>safety</strong> bulletin board near the unsafe or unhealthful reports toadvise personnel of the status of the complaint. Weeklynewsletters or plan of the day notices may be used in additionto or instead of posting.8. The completed response shall encourage, but not require, theoriginator to informally contact the <strong>safety</strong> office if theoriginator desires additional information or is not satisfiedwith the response. The response shall indicate formal appealsmay be made <strong>and</strong> provide appeal process reference.9003. HAZARD REPORT APPEALS1. If the originator of a NAVMC 11401, Unsafe or UnhealthfulWorking Conditions, or the NAVMC 11509, ANYMO<strong>US</strong>E report asdescribed in paragraph 9006, is dissatisfied with the finaldetermination or corrective action taken, the originator shouldfirst talk with the ISM or unit <strong>safety</strong> officer <strong>and</strong> attemptresolution. If, after a discussion with the <strong>safety</strong> manager orofficer, the originator remains dissatisfied, an appeal to thecomm<strong>and</strong>er shall be made in writing, describing in detail thehazardous condition to include the following:a. The OSH st<strong>and</strong>ard violated (if known).b. How, <strong>and</strong> to whom, the original report of the conditionwas given.c. What action resulted?d. An explanation of the dissatisfaction <strong>and</strong> anyrecommendation for correction.2. If first level of appeal response does not satisfy theoriginator of the report, additional appeals may be submitted.9-5
