14002 MARCOR OSH PROGRAM MANUALa. One copy shall be posted at the primary entrance to thespace. All other entry points shall be under control of theentry supervisor.b. One copy shall be provided to the supervisor requestingthe permit.c. One copy shall be retained by the CSPM for 1 year afterits expiration date. If the CSPM is not assigned to theinstallation or unit safety office, one copy shall be providedto that office.d. If the space is rejected, then a reject permit shall bewritten. One copy of the reject permit should be red or pinkwith ‘reject’ imprinted on it for high visibility.5. Training and Qualificationsa. The CSPM and ACSPM shall have successfully completed acourse that fully qualifies them to perform all the requirementsof 29 CFR 1910.146 and this Manual (training or travel costs arenormally provided at organizational expense). Recommendedcourses include the following or equivalent courses:(1) OSHA 2260, Permit-Required Confined Space Entry.(2) NAVOSHENVTRAINCEN A-493-0030, Confined Space Safety.b. When the training course is completed, the appointedCSPM and ACSPM should also receive 80 hours of on-the-jobtraining (OJT) from a qualified CSPM.c. Personnel found qualified because of their training andproven competency shall be recommended to the commander by theCSPM for formal certification and appointment as the ACSPM.Records of training and certification of competency shall bemaintained by the CSPM for as long as such personnel areinvolved in the program. All assistants shall be re-certifiedby the CSPM annually.d. The CSPM shall ensure attendants, when required, areinstructed in their duties, knowledgeable in the execution ofthe emergency alert system, and formally informed of their14-8
