MARCOR OSH PROGRAM MANUAL 14002restriction from entry into the confined space regardless ofcircumstance. Issuance of such instructions shall be noted onthe permit. Attendants shall be provided training in basicfirst aid <strong>and</strong> cardiopulmonary resuscitation (CPR). Attendant isan individual stationed outside one or more permit-requiredconfined spaces who monitors the authorized entrants <strong>and</strong>performs the attendant duties assigned in the installation’spermit space program.e. The CSPM shall ensure an emergency plan is written forthe installation’s permit space emergency rescue procedures (seeparagraph 14006).f. All affected personnel to include entry supervisorsshall be trained <strong>and</strong> certified IAW with 29 CFR 1910.146. Theresponsible manager or supervisor shall certify all authorizedentrants are trained on skills necessary for the safeperformance of assigned duties, including specific hazardslikely to be encountered as well as the appropriate <strong>safety</strong>equipment <strong>and</strong> <strong>safety</strong> measures. The CSPM shall assist in thistraining, as necessary, <strong>and</strong> seek the assistance of the RPPM,fire, <strong>and</strong> industrial hygiene personnel to assist in thistraining, where needed. The actual training <strong>and</strong> refreshertraining received by confined space entry workers must beformally included in their official training record. Theresponsible entry supervisor shall certify on the permit thatthe proposed entrants are properly trained in their duties underthis Manual <strong>and</strong> 29 CFR 1910.146.g. Control of energy sources, electrical <strong>and</strong> mechanical,shall be noted on the confined space permit <strong>and</strong> shall be lockedout or tagged out in accordance with chapter 12.h. Personnel assigned confined space entry duties shouldhave their duties included in their position description <strong>and</strong>/orpersonnel records. They shall receive a pre-placement physicalexamination based upon the type of work to be performed <strong>and</strong>potentially hazardous exposures. An annual physical examinationwill be given based on the exposure information compiled by theindustrial hygienist or medical examination protocol developedby medical officials. Termination examinations will be givenwhen personnel are permanently reassigned from confined spaceentry duties or leave the employment of the organization.14-9
