9002 MARCOR OSH PROGRAM MANUALadvise the work center supervisor of the condition noted, eitherorally or in writing. Initial oral reports are required forimminent danger situations. Reprisals against personnel forsubmitting hazard reports are prohibited.2. Upon receipt of an oral or written report of unsafe orunhealthful work practice or condition, the supervisorresponsible for the area in question shall investigate <strong>and</strong>initiate appropriate corrective action. When an unsafe orunhealthful work practice or condition cannot be corrected atthe supervisory level, the supervisor shall notify the ISM orunit <strong>safety</strong> officer within 5 working days.3. Any employee desiring anonymity may submit a NAVMC 11401,"Unsafe or Unhealthful Working Conditions" form, (figure 9-1) tothe ISM. Blank copies of this form <strong>and</strong> procedures shall beposted in or near all Marine Corps work centers (e.g., officialbulletin boards, time clocks). Military personnel may use theNAVMC 11401 or the ANYMO<strong>US</strong>E report as described in paragraph9006.4. If anonymity is requested, the ISM or unit <strong>safety</strong> officershall delete the originator's name <strong>and</strong> any individual named inthe report, <strong>and</strong> advise the responsible supervisor that ahazardous condition has been reported.5. The <strong>safety</strong> office shall record each report on a logmaintained within the installation or unit <strong>safety</strong> office. Asequentially numbered case file shall be assigned for purposesof maintaining an accurate record of the report. The log shallinclude the following: date, time, RAC code, reference, filenumber, location of condition, brief description of thecondition, classification (i.e., imminent danger, serious, orother), <strong>and</strong> date/action taken. See figure 9-2 for log pageexample.6. The <strong>safety</strong> office shall investigate all hazard reports.Alleged imminent danger situations shall be investigated within8 hours, potentially serious conditions within 3 working days,<strong>and</strong> other <strong>safety</strong> <strong>and</strong> health conditions within 20 working days.If the report involves a health hazard, the <strong>safety</strong> office shallrefer the hazard report to the local medical facility within oneday for investigation.9-4
