one show a positive relationship with smoking <strong>and</strong> a dose–response relationship<br />

(Fig. 26.3). The adjusted odds ratios for current smokers are generally higher than<br />

those reported for invasive cancer, ranging between 1.7 <strong>and</strong> 6.6. The reasons for this
