PRISMA-7 Questionnaire
To identify elderly people
with a moderate or severe loss of autonomy
Does this person have a case manager ?
If yes, send the information you have on the person’s health to the case manager, and do
not complete this questionnaire.
If this person does not have a case manager, do you know his/her « up-to-date » SMAF
score ?
If yes, do not complete this questionnaire. If his/her score is ≥ 15, refer the person’s file
to the single entry point.
This questionnaire is designed for elderly people who do not have a case manager and
whose SMAF score is not known.

Instructions
For questions 3 through 7, no interpretation of the answer is necessary. Simply note the
person’s answer without considering if the answer should be Yes or No. If the elderly person
hesitates between Yes and No, you must ask him/her to choose one of the two
answers. If, despite several attempts, he/she persists in answering "A bit" or "Sometimes",
check Yes.
