Et konsistent fund hos <strong>PTSD</strong>-partienter – med billeddannende teknikker (MRI <strong>og</strong> PETscanninger)<br />

– er mangler i hippocampal struktur <strong>og</strong> funktion. Bremner et al (377) har således<br />

fundet en mangel i hippocampal aktivering <strong>og</strong> 16% mindre volumen hos kvinder, s<strong>om</strong> havde<br />

været seksuelt misbrugt <strong>og</strong> led <strong>af</strong> <strong>PTSD</strong>, i forhold til kvinder, der havde været sammenligneligt<br />

seksuelt misbrugt uden at lide <strong>af</strong> <strong>PTSD</strong>. Kvinder med seksuelt misbrug <strong>og</strong> <strong>PTSD</strong> havde 19%<br />

mindre hippocampal volumen målt med PET-scanning, sammenlignet med en tilsvarende<br />

gruppe kvinder uden seksuelt misbrug <strong>og</strong> uden <strong>PTSD</strong>.<br />

Flere forskere har foreslået en model ud fra de mange neurovidenskabelige undersøgelser med<br />

billeddannende teknikker <strong>af</strong> <strong>PTSD</strong>. Figur 9 viser, at de centrale <strong>PTSD</strong>-sympt<strong>om</strong>er er medieret<br />

<strong>af</strong> en dysfunktion i de mediale præfrontale <strong>om</strong>råder, specielt i gyrus cinguli anterior, s<strong>om</strong><br />

bevirker en manglende modulering <strong>og</strong> inhibition <strong>af</strong> amygdala-aktivitet <strong>og</strong>/eller en indbygget<br />

lavere amygdala responstærskel/hyperarousal over for frygtfremkaldende stimuli. Overaktivering<br />

<strong>af</strong> amygdala orkestrerer en lang række <strong>PTSD</strong>-sympt<strong>om</strong>er 47 (377;378).<br />

“Amygdale hyperresponsivity mediates sympt<strong>om</strong>s of hyperarousal and explains the indelible quality<br />

of the emotional memory for the traumatic event; inadequate influence by the anterior cingulate<br />

cortex underlies deficits of habituation; and decreased hippocampal function underlies deficits in<br />

identifying s<strong>af</strong>e contexts, as well as acc<strong>om</strong>panying explicit difficculties. Further, we propose that in<br />

threatening situations, patients with <strong>PTSD</strong> exhibit an exaggerated reallocation of resources toward<br />

regions that mediate fight-and flight responses and away fr<strong>om</strong> widespread heter<strong>om</strong>odal cortical areas<br />

as a neural substrate for dissociation” ((379), p. 967).<br />

Modellen foreslår endvidere, at den øgede amygdala-aktivering medvirker til den førnævnte<br />

hippocampusatrofiering s<strong>om</strong> følge <strong>af</strong> sympt<strong>om</strong>erne på hyperarousal.<br />
