6.4 Ma<strong>la</strong>ria<br />

− Ma<strong>la</strong>ria was en<strong>de</strong>mic in Fin<strong>la</strong>nd during the period un<strong>de</strong>r study, mainly in the coastal<br />

areas. There were also some cases in<strong>la</strong>nd, but they were rarer. The clearly en<strong>de</strong>mic<br />

zones were located in the southwest of the country, especially the Å<strong>la</strong>nd Is<strong>la</strong>nds, but<br />

also the regions of Uusimaa, Itä-Uusimaa and Varsinais-Suomi.<br />

− Ma<strong>la</strong>ria was an important modu<strong>la</strong>ting factor of the health state of the popu<strong>la</strong>tion, as it<br />

could act weakening and predisposing people to suffer from other diseases. It caused<br />

less than one per cent of all <strong>de</strong>aths during the period un<strong>de</strong>r study, but consi<strong>de</strong>ring the<br />

low case-fatality rate of P<strong>la</strong>smodium vivax, the prevalent parasite in Fin<strong>la</strong>nd, the<br />

actual proportion of sick people must have been quite <strong>la</strong>rge.<br />

− The importance of ma<strong>la</strong>ria varied from year to year, not following a pre-<strong>de</strong>termined<br />

trend or external perturbations of the popu<strong>la</strong>tion such as wars or famines. Those<br />

periods of higher ma<strong>la</strong>ria inci<strong>de</strong>nce were probably a reaction to environmental<br />

changes in terms of temperature or humidity. Temperature was probably an important<br />

limiting factor: low temperatures implied low inci<strong>de</strong>nce of ma<strong>la</strong>ria but not necessarily<br />

the other way round.<br />

− The most affected age groups were children 1 to 4 years old and old people, which<br />

reinforces the i<strong>de</strong>a of en<strong>de</strong>mic ma<strong>la</strong>ria in the country.<br />

− Most of the ma<strong>la</strong>ria <strong>de</strong>aths took p<strong>la</strong>ce in spring, which is common in areas infected<br />

with the hibernans strain of P<strong>la</strong>smodium vivax. In the coastal areas a slightly different<br />

seasonality can be observed, with a minor sub-peak in autumn. This autumnal portion<br />

of ma<strong>la</strong>ria mortality could be the consequence of infections with P. ma<strong>la</strong>riae but it is<br />

more p<strong>la</strong>usibly re<strong>la</strong>ted to more subtropical strains of P. vivax.<br />

− During the period un<strong>de</strong>r study it can be observed a gradual process of stabilization of<br />

ma<strong>la</strong>ria in specific areas: during the <strong>la</strong>st half of the 18th century there were peaks of<br />

the disease in a <strong>la</strong>rger number of areas than during the first half of the 19th century.<br />

There is also a gradual reduction of the mean age of ma<strong>la</strong>ria <strong>de</strong>aths, even if the general<br />

average age at <strong>de</strong>ath tends to increase gradually.
