Outcome<br />

Many women were found to discontinue breastfeeding<br />

during the 1st few weeks post partum as a result of<br />

Perceived Insufficient Milk (PIM) supply. Many women<br />

used infant satisfaction cues (e.g. crying, unsettled) as their<br />

primary indicators of milk supply. The H & H Lactation<br />

Scale and the Perceived Insufficient Milk Tool have been<br />

found useful to identify women at risk during early postpartum.<br />

Furthermore, PIM was associated with early<br />

weaning and / or decreased exclusivity in 10 studies. PIM<br />

was also associated with lower self efficacy or maternal<br />

confidence scores. Use of <strong>for</strong>mula in hospital was<br />

associated with PIM in 3 studies and ceasing breastfeeding<br />

be<strong>for</strong>e leaving the hospital was related to PIM in 1 study.<br />

The majority of the studies <strong>review</strong>ed identified IM as PIM.<br />

Both of the terms have different definitions.<br />

Effect on duration with Prenatal Interventions: 6 of 8 studies<br />

effective (4 of 6 RCTs) with AF ranging from 19-78% <strong>for</strong> full<br />

BF at 6 mo to any BF at 4 wk.Effect on duration with Postnatal<br />

Interventions:3 of 9 studies effective (2 of 8 RCTs) with AF<br />

ranging from 15-53% <strong>for</strong> any BF at 2 mo to full BF at 6 mo.<br />

Effect on duration with BOTH Pre- and Postnatal Interventions:<br />

7 of 9 studies effective (3 of 4RCTs) with AF ranging from 20-<br />

92% <strong>for</strong> full BF at 4 mo in 2 studies; One study had an AF of<br />

91% <strong>for</strong> full BF at 5 mo. Effect on duration with both Hospital<br />

and Postnatal Interventions: 4of 7 studies effective (3 of<br />

6RCTs) with AF ranging from 24-88% <strong>for</strong> any BF at 4 mo to<br />

exclusive BF at 4 mo. Effect on duration with both Hospital and<br />

Pre- and Postnatal Interventions: All 4 studies effective (3<br />

RCTs) with AF ranging from 20- 100% <strong>for</strong> any BF at 1month<br />

to full BF at 6 mo.<br />
