Conclusion<br />

PIM is one of the most common and influential reasons <strong>for</strong><br />

low rates of breastfeeding duration and exclusivity. Future<br />

research should be conducted to determine who is at high<br />

risk and to further validate screening tools; and whether<br />

PIM is a physiological or psychological issue. Perceived<br />

milk supply is considered modifiable and well-in<strong>for</strong>med<br />

interventions to reduce the incidence of PIM might be a<br />

key element <strong>for</strong> improving rates of successful<br />

breastfeeding.<br />

The primary health care units should in<strong>for</strong>m, encourage, and<br />

support pregnant women in breastfeeding; the maternity<br />

hospitals should allow women to bond with their babies and<br />

help them to establish breastfeeding; and the primary health<br />

care units should be able to guide, rein<strong>for</strong>ce, and support this<br />

practice continuously, completing the cycle. Although there is<br />

evidence supporting the effectiveness of primary care strategies<br />

in extending breastfeeding duration, there is a need <strong>for</strong> broadbased,<br />

well-designed studies testing the effect of the<br />

combination of the procedures referred above, preferably<br />

spanning the prenatal and postnatal periods, to encourage the<br />

development of evidence-based protocols concerning the<br />

promotion, protection, and support of breastfeeding in primary<br />

care.<br />
