General Reports(a) Semiannual Operating Permit ReportsAny monitoring reports required by this permit to be submitted to the Puget SOtmd Clean AirAgency shall be submitted at least once every six months, or more frequently where requiredby an applicable requirement and received at Puget Sound Clean Ail' Agency 01' postmarkedno later than August 30 for period January I-June 30 and February 28 for period July 1-December 31. All instances of deviations from permit requirements must be clearlyidentified in such reports. If there were no deviation Boeing must submit a report statingthat, there were no deviations. [WAC 173-401-615(3)(a), 10117/02](b) Deviation Reports. Boeing shall report in writing to Puget Sound Clean Air Agency Operating PermitCertification all instances of deviations from the permit requirements, including thoseattributable to upset conditions as defined in tbis permit, the probable cause of the deviations,and any corrective actions or preventive measures taken. "Deviation" means any situation inwhich an emission unit fails to meet a permit term or condition. Boeing shall maintain acontemporaneous record of all deviations. Boeing shall report any deviations to the PugetSound Clean Air Agency that represent a potential threat to human healtb or safety by FAX(206-343-7522) soon as possible but no later than 12 hours after such a deviation isdiscovered. Boeing shall report other deviations in writing to Puget Sound Clean Air AgencyOperating Permit Certification on a monthly basis, within 30 days after the end of the monthin which the deviation is discovered. Boeing is not required to submit a monthly report formonths during which there were no deviations, except that if there are no deviations during acalendar half, Boeing must report that there were no deviations by August 30 for theBoeing-Everett Permit, issued September 12, 2008; Administrative Amendment 3: October 10, 2012

[WAC 173-401-615(3)(b), 10117/02; WAC 173-400-107,9/20/93](c) Reporting CertificationAny application form, report, or compliance certification that is required to be certified byany applicable requirement or is submitted pursuant to this permit shall contain certificationby a responsible official of truth, accuracy, and completeness. This certification and anyother certification required under this permit shall state that, based on information and beliefformed after reasonable inquiry, the statements and information in the document are true,accurate, and complete.
