[40 CFR Section 63.10(d), 4120106]Boeing shall submit immediate startup, shutdown and malfunction reports to the <strong>Puget</strong><strong>Sound</strong> <strong>Clean</strong> <strong>Air</strong> <strong>Agency</strong> if an action taken during a startup, shutdown or malfunction isnot consistent with the procedmes specified in the Startup, Shutdown and MalfunctionPlan and the somce exceeds any applicable emission limitation in the relevant standard.These reports shall be submitted within 30 days of the end of the month in which theaction occmred. Method 9A ReportsBoeing shall report to the <strong>Puget</strong> <strong>Sound</strong> <strong>Clean</strong> <strong>Air</strong> <strong>Agency</strong> results of all opacity monitoringusing Ecology Method 9A within 30 days after the end of the month that the measmementoccurred. These reports will be certified in accordance with V.Q.1.(c) at least semi-annually.[WAC 173-401-615(3)(a), 10117/02]5. Report of Problems not Corrected Within 24 HoursIf Boeing is reporting a problem (such as leak, out of range pressure drop, out of range pH, orother problem, as applicable) in lieu of correcting it or shutting down the associatedequipment or activity in accordance with Sections II.A.1(b), II.A.1(c), II.A.1(d), II.A.1(f),11.A.2(d)(ii), 11.A.2(d)(v), Error! Reference source not found., Error! Reference sourcenot found., Error! Reference source not found., Error! Reference source not found. thenBoeing shall report to the <strong>Agency</strong> in writing by facsimile (206-343-7522) to <strong>Puget</strong> <strong>Sound</strong><strong>Clean</strong> <strong>Air</strong> <strong>Agency</strong> Attn.: <strong>Operating</strong> <strong>Permit</strong> Certification, the nature of the problem andBoeing'S intent to continue operating while seeking to address the problem.In addition, within 30 days after the end of the month in which the problem was reportedunder this section (V.Q.5), Boeing shall also submit either:(a) A deviation report pmsuant to V.Q.1(b) Deviation Reports; or(b) A report indicating that after reasonable inquiry Boeing has determined that nodeviation occmred and the basis for that determination.All reports submitted pmsuant to this Section V.Q.5 shall be certified in accordance withBoeing-Everett <strong>Permit</strong>, issued September 12, 2008; Administrative Amendment 3: October 10, 2012
