Regulation I, Article 12Boeing shall submit a monthly report to Puget Sound Clean Air Agency within 30 days afterthe end of the month in which the data were recorded. This report shall include:• The date, time period, magnitude (in the units of the standard) and cause of each emissionthat exceeded an applicable emission standard;• The date and time of all actions taken to correct the problem, including any actions takento minimize the emissions during the exceedance and any actions taken to prevent itsrecurrence;• The number of hours that the equipment (required to be monitored) operated each monthand the number of valid hours of monitoring data that the monitoring system recoveredeach month;• The date, time period, and cause of each failure to meet the data recovery requirements ofSection 12.03(b) and any actions taken to insure adequate collection of such data;• The date, time period, and cause of each failure to recover valid hourly monitoring datafor at least 90% of the hours that the equipment (required to be monitored) was operatedeach day;• The results of all cylinder gas audits conducted during the month; and• A certification of truth, accuracy, and completeness signed by an authorizedrepresentative ofthe owner or operator.[Puget Sound Clean Air Agency Reg I: 12.03, 4/9/1998, Order of Approval No. 7438Condition 4, 8/4/99] [Puget Sound Clean Air Agency Reg I: 12.03,9123104, State Only]9.
