The disclosure is required for all contrcts in excess of$17,500 that are not awarded purst toa ''fair and open" process eN.J.S.A. 19:44A-20.7).2. Due to the potential lengt of ~ome contractor submissions, the public agency should considerallowing data to be submitted in elecnic form (i.e., spreadsheet pdf fie, etc.). Submissionsmust be kept ~ the contrct docuieiits or in an appropriate computer fie and be available forpublic access. The form is worded to accept thi alternate submision. The text should beamended if electromc submission wi not be allowed.on fie. ",4. The contrctor must disclose contrbutions made to cadidate and par commtt coveng a3. The Division ofLoal Governent Servces recommends that contractors be provided a list of the afecedagenC?ies. Ths wil assist contrctors in determnig the campaign and politcal committees oftheoffcials and cadidates afected by the disclosure.a. The Division has prepar model disclosure fonns for each cOunty. They Ca bedownloaded frm the "County PCD Forms" link on the Pay-to-Play web site atww.nj.gov/dcalgsp2p. They wil be updte frm tie-to-tie as necsar.b. .A public agecy using thes forms šhould edit them to properly reßed the corrtleglatie distrct(s). As the forms are county-bas~ they list alllegilative district ineach comity. Distrcts that do not represent the public agency should be removedfrom the lits. . .c.
