It is not intendedto be provided to contractors. What follows are instrctions on the use of foin local uiuts canprovide to contractors that are required to disclose political contributions pursuant to N.J.S.A.19:44A-20.26 (p.L. 2005, c. 271, s.2)~ Additional infonnation on the process is available inLocal Finance Notice 2006-1 (ww.nj.gov/dcalgslfnllfnmenu.shtm).1. The disclosure is required for all contracts in excess of$t 7,500 that are not awarded.process (N.J.S.A. 19:44A-20.7)..a "fair and open"pursuant to2. Due to the potential length of some contractor submissions, the public agency should considerallowing data to bè submitted in electronic fonn (i.e., spreadsheet, pdffie, etc.): Submissionsmust be kept with the contract documents or in an appropriate computer fie andbe available forpublic access. As the forms are county-bas~d, they list ali legislative districts ineach county. Distrcts that do not represent the public agency should be removed(rom the lists.c. Some contractors may find it easier to provide a single list that covers all contributions,the county. These submissions are appropriate and should be accepted.regardless ofd. TIie form may be used "as-is", subject to edits as described herein.e. The "Contractor InstctionsH sheet is intended to be provided with the form. It isrecommended that the Instictions and the fonn be printed on the same piece of paper..The form notes that the Intrctions are printed on the back of the fonn; where that is notthe case, the text should be edited accordingly.f. The form is a Word document and can be edited to mèet loca needs, and posted.fordownload onweb sites, used as an e-mail attachment, or provided as a pnnted document..:1.5. It is reonimendedthat the contrctor also complete a "Stockholç1er Disclosure Certification."This wil assist the local unit in its obligation to ensure that contrctor did not make anyprohibited contrbutions to the commitú:es listed on the Business Entity Disclosure Certjficationin the 12 months pnor to the contrct.
