The disclosure is required for all contrcts in exces of $17,500 that are not awarded pursant toa "fair and open" process (N.L.S.A. 19:44A-20.7). .2. Due to the.potential lengt of ~ome contractor submissions, the public agency should considerallowIng datato be submitted in elecnic form (i.e., spreadsheet, pdf fie, etc.). Submissionsmust be kept with the contract documents or in an appropriate computer fie and be available forpublic access. The.ronn is worded to accept tJW alternate submision. The Division ofLocal Governent Services recommends that contractors be provided a Jist of the afectedagen~ies. This will assist contrctors in determg the campaign and political committees ofoffcials ànd candidates afected by the diclosure.a. The Division has prepar model discloaue fOnDS for each county. It is remmended that the contrctor also complete a "Stockhol~er Disclosue Certifcation."'.
