..c. 21-1 POLmCALCONIBUTON DISLOSURE fORMContractor InstructionsBusiness entities (contractors) receiving contracts froi: a public agency that ar NOT awar(led puruant toa "fair and open" process (defined at NJ.S,A. 19:44A-20.7) are subject to the provisions ofP.L. 200S, c.271, s.2 (M.J.S.A. 19:44A-20.26).
