4 LEAVE4.11.5.2 Recredit <strong>of</strong> Extended Leave – Minimum PeriodIn the case <strong>of</strong> extended leave, the illness <strong>and</strong> supporting medical certificates must befor a period <strong>of</strong> at least five consecutive working days before the leave will berecredited.4.11.5.3 Leave Prior to Resignation or TerminationThese provisions do not apply to staff members on leave prior to resignation ortermination <strong>of</strong> services, unless the resignation or termination <strong>of</strong> services amounts toa retirement.4.11.6 MEDICAL CERTIFICATES FOR SICK LEAVE PURPOSES4.11.6.1 GeneralA staff member shall provide a medical certificate for any absence in excess <strong>of</strong> threedays or for a lesser period if required. See also Section 4.11.10 Sick LeaveMonitoring.4.11.6.2 Medical Certificate for Part <strong>of</strong> an Absence OnlyWhere a staff member has been absent for a period exceeding three days <strong>and</strong>presents a medical certificate which covers the latter part <strong>of</strong> the absence only, sickleave shall be granted for the entire period provided the Director-General is satisfiedthat the reason for the absence is genuine.4.11.6.3 Acceptability <strong>of</strong> Certificatea) A certificate issued by a registered medical practitioner shall be accepted for sickleave purposes.b) A certificate issued by a registered:dentist;optometrist;chiropractor;osteopath;physiotherapist;oral <strong>and</strong> maxilla facial surgeon who is not also a registered medicalpractitioner;shall be accepted for a maximum period <strong>of</strong> one week.Periods in excess <strong>of</strong> one week must have the support <strong>of</strong> a registered medicalpractitioner.4.11.6.4 Medical Certificates to be Submitted to HealthQuestWhere a staff member applies for sick leave <strong>and</strong>:a) no medical condition has been disclosed on the medical certificate; orb) the staff member wishes the nature <strong>of</strong> illness to remain confidential – referH<strong>and</strong>book for non teaching staff in schools 2007 Page 4 - 53
