4 LEAVE4.11.8.3 Medical Certificate RequiredThe staff member shall submit an application for sick leave with:a) the medical certificate in a sealed envelope; <strong>and</strong>b) a request for the nature <strong>of</strong> the illness to remain confidential;for referral to the Director <strong>of</strong> HealthQuest for determination as to whether a grant <strong>of</strong>sick leave is warranted.Under no circumstances shall the sealed envelope be opened within the <strong>Department</strong>.4.11.8.4 Disclosure <strong>of</strong> Nature <strong>of</strong> IllnessOnly in exceptional circumstances will the Director <strong>of</strong> HealthQuest disclose to theDirector-General the nature <strong>of</strong> the illness. Those circumstances would be wherescreening or counselling <strong>of</strong> other staff members or the public may be required as aresult <strong>of</strong> having contact with the staff member who has applied for the leave.Where this occurs, there shall be full consultation between the staff member, the staffmember's treating doctor(s) <strong>and</strong> the Director <strong>of</strong> HealthQuest.4.11.9 HEALTH RISKS TO THE INDIVIDUAL STAFF MEMBER, OTHER STAFF MEMBERSAND/OR THE PUBLIC4.11.9.1 GeneralWhere a staff member:a) is known to have had contact with someone suffering from an infectious disease;orb) enters on duty or resumes duty after suffering tuberculosis; orc) is considered to pose a significant health risk in any other way, to that staffmember, other staff members <strong>and</strong>/or the public;the Director-General shall immediately contact the Director <strong>of</strong> HealthQuest for adviceas to the procedure to be adopted.4.11.9.2 Direction to Cease DutyThe Director-General may direct such staff member to cease duty or not to resumeduty in the following circumstances:a) where the advice <strong>of</strong> the Director <strong>of</strong> HealthQuest is that the staff member shouldnot be at work pending investigation <strong>and</strong>/or examination; orb) where the Director-General considers that there may be a health risk to the staffmember, other staff members <strong>and</strong>/or the public, by that staff member remainingon duty for the time it may take to obtain the advice <strong>of</strong> the Director <strong>of</strong>HealthQuest.H<strong>and</strong>book for non teaching staff in schools 2007 Page 4 - 55
