4 LEAVEii)iii)a medical certificate which includes anticipated date <strong>of</strong> birth or in thecase <strong>of</strong> an adopted child, documentation from the authority indicatingthe expected date <strong>of</strong> taking custody; <strong>and</strong>where the application includes a period <strong>of</strong> paid leave, the staffmember must specify the number <strong>of</strong> weeks required at full pay <strong>and</strong>half pay or whether payment is requested in a lump sum.(NB: While one month’s notice is the minimum, up to three months’notice is advisable particularly where lump sum payment in advanceis requested).iv)where a staff member has determined that they wish to requestadditional leave as per Section 4.2.6.3, prior to commencing leave theyare encouraged to apply for the whole period at once. No additionalapplications or approvals are required.v) where a staff member intends to use accrued extended leave tosupplement half pay or unpaid adoption, maternity or parental leave thisshould be included in the application.b) Variation <strong>of</strong> leavei) A staff member may vary the intended period <strong>of</strong> leave any number <strong>of</strong>times before it commences.ii)iii)After leave commences the period <strong>of</strong> leave may be varied oncewithout prior approval <strong>and</strong> on any number <strong>of</strong> subsequent occasionswith approval. However resumption <strong>of</strong> duty must then coincide withthe commencement <strong>of</strong> a school term. In all cases, a staff membermust provide at least four weeks’ notice during term time bysubmitting an amended leave request to the principal or theirnominee.Subject to approval, a staff member who has returned to duty afterleave, may, provided the staff member has a balance <strong>of</strong> adoption,maternity or parental leave to credit, apply to revert to full time or parttime unpaid adoption, maternity or parental leave.c) Communication while on leavei) A staff member must take all reasonable steps to inform the <strong>Department</strong>about any significant matter that will affect the staff member’s decisionregarding the duration <strong>of</strong> leave to be taken, whether the staff memberintends to return to work <strong>and</strong> whether the staff member intends torequest to return to work on a part time basis.ii)A staff member shall also notify the <strong>Department</strong> <strong>of</strong> changes <strong>of</strong> addressor other contact details which might affect the <strong>Department</strong>’s ability tocomply with Section 4.2.6.1(c).d) Resumption <strong>of</strong> dutyOne month during term time prior to the date on which a staff member is dueH<strong>and</strong>book for non teaching staff in schools 2007 Page 4 - 13
