SECTION 013526 - GOVERNMENTAL SAFETY REQUIREMENTS: continued<br />

1.12 REPORTS:<br />

A. Accident Reports:<br />

1. Conduct an accident investigation for recordable injuries and illnesses, as defined in 1.3.h<br />

and property damage accidents resulting in at least $2,000 in damages, to establish the<br />

root cause(s) <strong>of</strong> the accident, complete the Navy Contractor Significant Incident Report<br />

(CSIR) from USACE Accident Report Form 3394 and provide the report to the<br />

Contracting Officer or Designated Representative within 5 calendar days <strong>of</strong> the accident.<br />

The Contracting Officer or Designated Representative will provide copies <strong>of</strong> any required<br />

or special forms.<br />

2. Conduct an accident investigation for any weight handling equipment accident (including<br />

rigging gear accidents) to establish the root cause(s) <strong>of</strong> the accident, complete the WHE<br />

Accident Report (Crane and Rigging Gear) form and provide the report to the<br />

Contracting Officer or Designated Representative within 30 calendar days <strong>of</strong> the<br />

accident.<br />

a. Do not proceed with crane operations until cause is determined and corrective<br />

actions have been implemented to the satisfaction <strong>of</strong> the Contracting Officer or<br />

Designated Representative.<br />

b. The Contracting Officer or Designated Representative will provide a blank copy <strong>of</strong><br />

the accident report form.<br />

B. Accident Notification:<br />

1. Immediately contact local emergency services organizations (911) for assistance as<br />

needed.<br />

2. Notify the Contracting Officer or Designated Representative as soon as practical, but not<br />

more than four hours, after any accident meeting the definition <strong>of</strong> Recordable Injuries or<br />

Illnesses or High Visibility Accidents, property damage equal to or greater than $2,000,<br />

or any weight handling equipment accident in accordance with NASA NPG 8621.1.<br />

3. Within notification include:<br />

a. Contractor name.<br />

b. Contract title.<br />

c. Type <strong>of</strong> contract.<br />

d. Name <strong>of</strong> activity.<br />

e. Installation or location where accident occurred.<br />

f. Date and time <strong>of</strong> accident.<br />

g. Names <strong>of</strong> personnel injured.<br />

h. Extent <strong>of</strong> property damage, if any.<br />

i. Extent <strong>of</strong> injury, if known.<br />

j. Brief description <strong>of</strong> accident (to include type <strong>of</strong> construction equipment used, PPE<br />

used, etc.).<br />

4. Preserve the conditions and evidence on the accident site until the Government<br />

investigation team arrives on-site and Government investigation is conducted.<br />

C. Crane Reports: Submit crane inspection reports required in accordance with USACE EM 385-<br />

1-1, Appendix I and as specified herein with Daily Reports <strong>of</strong> Inspections.<br />

D. Certificate <strong>of</strong> Compliance:<br />

1. Provide a Certificate <strong>of</strong> Compliance for each crane entering an activity under this<br />

contract (see Contracting Officer or Designated Representative for a blank certificate).<br />

2. <strong>State</strong> within the certificate that the crane and rigging gear meet applicable OSHA<br />

regulations (with the Contractor citing which OSHA regulations are applicable, e.g.,<br />

cranes used in construction, demolition, or maintenance comply with 29 CFR 1926 and<br />

USACE EM 385-1-1 Section 16 and Appendix I.<br />
