2-0700-6 Adjudication<br />

6. Adjudication.<br />

(1) Death certificate.<br />

(2) Name(s) and address(es) <strong>of</strong> next <strong>of</strong> kin.<br />

(3) Marriage certificate (civil certificate).<br />

(4) Birth certificate for each child (to show the legal relationship upon which the claim is<br />

based).<br />

(5) Divorce, dissolution, or death certificate for prior marriages.<br />

(6) Itemized burial bills, receipted if paid (see paragraph 14).<br />

a. If the record shows that any <strong>of</strong> the five basic requirements with regard to the employee's<br />

death is not satisfied, or the claimant is ineligible for benefits, the CE will prepare a formal decision<br />

(see <strong>FECA</strong> PM 2-1400). Form CA-1079 is used to transmit the decision. If the case satisfies the<br />

five basic requirements and there is at least one eligible beneficiary, the CE will:<br />

(1) Complete Form CA-674, which is a checklist with a section which applies specifically<br />

to death cases. The form is a permanent part <strong>of</strong> the record and should remain at the top <strong>of</strong><br />

the file.<br />

(2) Obtain the concurrence <strong>of</strong> the SCE in accepting the case as indicated by certification<br />

<strong>of</strong> the CA-674.<br />

(3) Initial and date Form CA-800 to indicate the acceptance.<br />

(4) Determine whether a dual benefits situation exists with respect to benefits from the<br />

Office <strong>of</strong> Personnel Management or the Veterans Administration (VA) and take appropriate<br />

steps (see <strong>FECA</strong> PM 2-1000).<br />

(5) Complete Form CA-24, <strong>FECA</strong> Fatal Benefit Payment worksheet, to authorize<br />

payment <strong>of</strong> applicable benefits if no dual benefits situation or other impediment to<br />

payment exists.<br />

(6) Advise the claimant <strong>of</strong> the acceptance by narrative letter, which should contain<br />

information regarding:<br />

(a) Four-weekly and monthly compensation entitlement for each eligible<br />

beneficiary (see paragraphs 7 through 10. The amount should be certified before<br />

release <strong>of</strong> the letter.)<br />

(b) "Lump Sum" provision upon remarriage, if applicable (see paragraph 7).<br />
