(3) Subjective complaints <strong>of</strong> the claimant.<br />

(4) Results <strong>of</strong> any laboratory tests and/or X-rays.<br />

(5) Identification <strong>of</strong> any pre-existing condition or underlying degenerative processes.<br />

(6) Diagnosis.<br />

(7) Prognosis.<br />

(8) Reasoned opinion as to the causal relationship between the diagnosed condition(s)<br />

and the employment incident or factors.<br />

(9) Course <strong>of</strong> treatment to be followed.<br />

(10) Clinical estimate <strong>of</strong> the date <strong>of</strong> partial and/or full recovery.<br />

(11) Any medically warranted work limitations (using the OWCP-5).<br />

If any <strong>of</strong> this information is lacking or requires clarification, the CE should ask the attending<br />

physician to furnish it. Failure <strong>of</strong> the attending physician to respond completely (or at all) within<br />

45 days indicates the need for a second opinion referral.<br />

d. Take the following actions:<br />

(1) Request additional medical evidence if necessary and establish the next intervention<br />

point. If the physician has not provided a comprehensive report as described above, a<br />

follow-up request should be made. If the physician has not yet given a clinical estimate <strong>of</strong><br />

the date <strong>of</strong> partial or full recovery, the CE should request this information and set a 45-day<br />

call-up.<br />

If complete and supportive medical evidence is received, establish the next intervention<br />

point and set a new call-up. This process should be repeated as necessary. If at any point<br />

a request for clarification does not result in receipt <strong>of</strong> the desired information within 45<br />

days, consideration should be given to referral for a second opinion.<br />

(2) Ask the employing agency to provide a copy <strong>of</strong> the claimant's position description, an<br />

assessment <strong>of</strong> the physical and any special emotional requirements <strong>of</strong> that position, and a<br />

statement as to the availability <strong>of</strong> limited or light duty. This information can be requested<br />

using Form CA-1656.<br />
