(5) Advising All Parties. The CE is responsible for notifying the claimant <strong>of</strong> unresolved<br />

issues which, if not satisfied, will lead to denial <strong>of</strong> the claim; notifying the agency <strong>of</strong> the<br />

disposition made <strong>of</strong> controverting evidence submitted by the agency; and notifying both<br />

claimant and agency <strong>of</strong> OWCP's decision in all denied cases and all cases where the claimant<br />

has not returned to work when the decision is made.<br />

2-0800-4 Forms<br />

4. Forms. A primary case must contain one <strong>of</strong> the notice forms listed below, and may include one or<br />

more <strong>of</strong> the claim forms. The form must identify the claimant, the employing agency, and the date <strong>of</strong><br />

injury.<br />

a. Forms Used.<br />

(1) In disability cases, the appropriate forms are:<br />

(a) Form CA-1, Federal Employee's Notice <strong>of</strong> Traumatic Injury and Claim for<br />

Continuation <strong>of</strong> Pay/Compensation.<br />

(b) Form CA-2, Federal Employee's Notice <strong>of</strong> Occupational Disease and Claim for<br />

Compensation.<br />

(c) Form CA-7, Claim for Compensation on Account <strong>of</strong> Traumatic Injury or<br />

Occupational Disease.<br />

(2) In death cases, the appropriate forms are:<br />

(a) Form CA-5, Claim for Compensation by Widow, Widower, and/or Children.<br />

(b) Form CA-5b, Claim for Compensation by Parents, Brothers, Sisters, Grandparents, or<br />

Grandchildren.<br />

(c)<br />

Form CA-6, Official Superior's Report <strong>of</strong> Employee's Death.<br />

b. Completion <strong>of</strong> Forms. A case which is created and entered into the data base may<br />

nonetheless lack appropriate forms. If either Form CA-1, CA-2 or CA-2a are incorrectly<br />

submitted, the CE must obtain the appropriate notice <strong>of</strong> injury or disease. Submission <strong>of</strong> an<br />

incorrect form is a technical error. It is improper to deny a case on the basis that the<br />

claimant failed to submit the correct form. The CE must obtain the appropriate notice <strong>of</strong><br />

injury or disease if it does not already appear in file.<br />

(1) It is sometimes impossible to locate an <strong>of</strong>ficial who has the authority, records, or<br />

knowledge <strong>of</strong> the facts to complete the reverse side <strong>of</strong> a notice <strong>of</strong> injury or claim form. This<br />

situation may arise due to a delay in reporting the injury and/or deactivation <strong>of</strong> the<br />

employing establishment. In such cases the CE should contact the Federal Records Center<br />

to obtain any information available (see paragraph 8 below).<br />

(2) When the notice <strong>of</strong> injury or claim for compensation is received directly from the<br />
