are correct and that continuing entitlement is substantiated in the file.<br />

a. Placement. When periodic roll payments are initiated, the CE should advise the claimant by<br />

Form CA-1049 or equivalent narrative letter. The letter should notify the claimant <strong>of</strong> the conditions<br />

for termination <strong>of</strong> benefits without prior notice and the requirement that employment be reported.<br />

A copy <strong>of</strong> the notice should be sent to the employing agency regardless <strong>of</strong> the claimant's current<br />

employment status.<br />

b. Monitoring. The CE will document the Periodic Entitlement Review (PER) annually, and all<br />

cases on the periodic roll will be monitored closely to:<br />

(1) Verify continuing entitlement to compensation and the appropriate level <strong>of</strong><br />

payments.<br />

(2) Ensure quality and appropriateness <strong>of</strong> medical care.<br />

(3) Reduce or terminate compensation payments when a claimant recovers from the<br />

employment-related condition or returns to work.<br />

(4) Initiate vocational rehabilitation and reemployment action as soon as it appears that<br />

permanent impairment may result or a change <strong>of</strong> job duties may be required due to the<br />

work-related injury.<br />

2-0812-5 Definition and Frequency <strong>of</strong> Reviews.<br />

5. Definition and Frequency <strong>of</strong> Reviews. All cases on the periodic roll require completion <strong>of</strong><br />

Forms CA-1032 and SSA-581 on a yearly basis. The <strong>of</strong>fice generates and mails Form CA-1032 and Form<br />

CA-935 (with enclosure SSA-581) to claimants. The CE should then review each case after 30 days has<br />

elapsed and complete the PER.<br />

Medical reviews should be accomplished in accordance with the case status as follows:<br />

a. PR. Cases in which temporary total disability payments are being paid require medical<br />

review once a year.<br />

b. PW. Cases in which payments are being made for a loss <strong>of</strong> wage-earning capacity require<br />

medical review every two years.<br />

c. PN. Cases in which the CE has determined, and the Supervisory Claims Examiner (SCE)<br />

has verified, that no wage-earning capacity exists require medical review every three years.<br />

d. PS. No medical reports need be requested in a case where a schedule award is being paid.<br />

These cases require annual release <strong>of</strong> Form CA-1032 where payment extends beyond one year to<br />
