(d) A suggested supplier, if any.<br />

(2) From the claimant:<br />

(a) The full name and address <strong>of</strong> two or three suppliers:<br />

(b) From each potential provider, a signed statement describing in detail the<br />

basic, unadorned item meeting the physician's specifications, and a breakdown <strong>of</strong> all<br />

costs, including delivery, installation, etc. If the claimant wants a more elaborate<br />

item (e.g., queen size bed vs. single needed for claimant), advise claimant to submit<br />

price quote for basic item only and <strong>of</strong> responsibility to pay for any enhancements.<br />

c. Review <strong>of</strong> Request. Evaluate the information received from the physician, claimant,<br />

service/equipment provider, and (if appropriate) RN or Rehabilitation Counselor. The CE may<br />

approve requests for health club/spa memberships, special equipment, supplies and services<br />

(except as noted in (2) below), provided the cost does not exceed $500. Requests involving higher<br />

amounts will be referred to the Supervisory Claims Examiner with a written recommendation<br />

explaining the basis for approval. For vehicle and house modifications, see PM Chapter 2-1800.<br />

(1) If the information received adequately supports the request, advise claimant by<br />

letter <strong>of</strong> the approval and any payment limitations or other restrictions or terms, e.g., rental<br />

vs. purchase, period <strong>of</strong> membership approved.<br />

(a) Only individual (not family or group) health club or spa memberships may be<br />

approved, and only for periods <strong>of</strong> six months at a time. The claimant should be<br />

advised <strong>of</strong> the period <strong>of</strong> approval, and if a further period <strong>of</strong> approval is requested, <strong>of</strong><br />

the need for a medical report (approximately 45 days prior to expiration <strong>of</strong> current<br />

membership term) explaining the gains achieved to date and supporting an<br />

extension for a specified period.<br />

(b) Nonpersonal equipment which costs more than $5000 and is not required as<br />

part <strong>of</strong> the approved vocational rehabilitation program is government property, and<br />

the claimant must be advised <strong>of</strong> this fact and informed that the item may not be<br />

resold, given away, traded, or otherwise disposed <strong>of</strong> without OWCP approval. The<br />

MMA should be advised <strong>of</strong> the purchase so that the required record can be<br />

established. See PM Chapter 3-400.4b.<br />
