through a warehouse roof. Other than that, there is just a bundle of steel<br />

and ceramic in the parking lot with some damage to the enclosure wall and<br />

fence.<br />

I won't go into details on the single point failure at this time, other<br />

than to say that the tower crew did NOT cause this accident. The third<br />

level guys were slacked at the time of the failure.<br />

ADDED EYEWITNESS INFORMATION<br />

The injured worker was atop the elevated guy anchor using a come-along to<br />

take up the slack of the north/east, third level guy. There were two<br />

workers at this location. The other two anchors are not elevated and were<br />

manned by two workers each.<br />

The three teams were working in unison and in radio contact with each<br />

other using come-alongs to slowly and evenly bring up the third level<br />

guys. The guys had just cleared the second level guys and [were] still<br />

slacked when the elevated anchor system failed. The injured worker jumped<br />

or fell to the ground and hurt his back. He was up and walking around but<br />

was taken to the hospital to get checked out.<br />

There was minor damage to a couple of truck trailers and part of the gin<br />

pole sheared and went through the roof into the warehouse. The rest of the<br />

damage was to the enclosure block wall and a fence. The major damage was<br />

to the tower only.<br />

Groundwave propagation on mediumwave: This statement<br />

"At 0.5 MHz over average ground, the ground wave predominates over the sky<br />

wave from the transmitter site out to distances of about 150 km, where the<br />

two signals are equal. The signals add as vectors, and destructive and<br />

constructive interference can occur. At distances beyond 150 km the sky<br />

wave is the predominant signal. At a signal frequency of 1.5 MHz, the<br />

distance where the two signals are equal reduces to 45 km, because of the<br />

increased loss at the higher frequency."
