We hereby submit our proposal to furnish labor; materials and equipment necessary to provide the
following work as per the information that you provided and as noted and described herein:
REOUEST FOR CHANGE ORDER
This proposal includes, and is limited to the following. Install thirt-six (36) tie backs near CMU walL.
Pour twenty-nine (29) concrete footings and install twenty-nine (29) tie backs as discussed during
site meeting. Set two (2) 3" gate posts in concrete footings and fabricate and install one (1) single
swing pedestrian gate. Weld twelve (12) existing finals back to existing posts. . Weld existing ciosers
and add Magna latch to existing swing gate. Install drop rods and hold backs on basketball and trash
enclosure gate. Remove and deliver wicket fence to Jersey City D.P.w. compound, and fill holes with
non shrink grout. And perform minor painting to existing welds on iron fence currently on site discussed
during site meeting 6/9/11.
Guardian Fence Co. Inc wil complete this work for the sum of $29,980.00 Plus 7% NJ sales tax if applicable
Note: Price is based on start date of 6/13/11.

This proposal is a conditional offer and is subject to the following checked items:
1) This proposal is valid for 15 days.

Clearing, grading, grubbing and/or removal of debris.
ayout, locating of propert lines, grades and related work.

M Padlocks for gates. All gates include provisions for padlocking. .
