VI. Special Grant Conditions.<br />

A. Subgrantee may be considered "high risk" if OHSP determes that a Subgrantee meets any ofthe<br />

following criteria.<br />

1. Has a hitory of unsatisfactory performance.<br />

2. Is not fiancialy stable.<br />

3. Has a fiancial management system which does not meet the standards set forth in section<br />

VIII of this agreement.<br />

4. Has not complied with term and conditions of a previous award.<br />

B. If a Subgrantee shaU be deemed to be a high rik by OHSP, but in its discretion OHSP dètennes<br />

that an award wil be made; special conditions and/or restrictions shall correspond to the high risk<br />

condition and shall be included in the aWard. Special conditions/restrictions may,include, but may be<br />

lited to, any of the following activities.<br />

1. Withholding authority to proceed until evidence of acceptable perfonnance withi a given<br />

funding period is observed.<br />

2. Requirg additionai more detailed fiancial reports.<br />

3. Additional project monitoring.<br />

4. Requiring the Subgranteeto obtain techncal or management assistance.<br />

5. Establihig additional prior approvals.<br />

C. IfOHSP imposes such conditions, the OHSP Grant and Program Management Bureau Chiefshall<br />

prepare written notification, as soon as possible, providing the following information.<br />

1. The_!1ature 0 f the special conditions/rèstrictions.<br />

2. The reason.(s) for imposing the special Conditions.<br />

3. The corrective actions that must be taken before the special conditions wil be removed by<br />

OHSP and the time alowed for completing the corrective actions.<br />

4. The method of requesting reconsideration of the conditions/restrictions imposed.<br />

D. The Subgrantee agrees to maintain, at its own expense, all ofthe equipment purchased with grant<br />

funds.<br />
