Because blood and fluids flow together, blood stasismay further become complicated phlegm obst<strong>ru</strong>ction andnodulation.TREATMENT BASED ON PATTERNDISCRIMINATION:1. WIND COLD DAMP IMPEDIMENT PATTERNMAIN SYMPTOMS: This pattern typically presents at the initialstage or when this disease’s course has not gone on toolong. Either one or many joints are swollen and painful.However, the affected area is not hot or red and may, in fact,feel cool or cold. <strong>The</strong>re is early morning stiffness, heaviness,numbness, and/or restricted movement. When the painobtains cold, it gets worse; when it obtains heat, it gets better.<strong>The</strong>refore, the pain typically follows changes in weather.Other symptoms include possible aversion to cold, loosestools, and clear, long urination. In the initial stage, fever,chills, and absence <strong>of</strong> sweating are possibly accompanied bysevere pain in the joints. <strong>The</strong> tongue is pale with thin, whitefur, and the pulse is deep and bowstring or deep and tight.NOTE: This pattern is not commonly seen in its pure formin <strong>Western</strong> patients who tend to have spleen vacuity and liverdepression in addition to wind cold damp impediment. Infact, it is a defensive qi vacuity due, in turn, to spleen qivacuity which allows wind, cold, and/or damp evils toinvade, resulting in impediment.TREATMENT PRINCIPLES: Expel wind and dispel dampness,warm the channels and scatter coldRX: Wen Jing Juan Bi Tang Jia Jian (Warm the Channels &Alleviate Impediment Decoction with Additions &Subtractions)INGREDIENTS:Dang Gui (Radix Angelicae Sinensis)Gui Zhi (Ramulus Cin<strong>nam</strong>omi)Yin Yang Huo (Herba Epimedii)ginger-processed Ban Xia (Rhizoma Pinelliae)Lu Xian Cao (Herba Pyrolae)processed Chuan Wu (Radix Aconiti Carmichaeli)processed Cao Wu (Radix Aconiti Kusnez<strong>of</strong>fi)Tu Bei Chong (Eupolyphaga/Steleophaga)Wu Shao She (Zaocys)Feng Fang (Nidus Vespae)Gan Cao (Radix Glyycrrhizae)20g15g15g15g9g9g9g9g9g9g5gANALYSIS OF FORMULA: Gui Zhi courses wind, warms thechannels, and scatters cold. Wu Shao She courses wind, freesthe flow <strong>of</strong> the network vessels, and stops pain. Ban Xia driesdampness. Yin Yang Huo, Chuan Wu, and Cao Wu course andeliminate wind and dampness, scatter cold and stop pain.Dang Gui and Tu Bie Chong quicken the blood and transformstasis, free the flow <strong>of</strong> the network vessels and stoppain. Gan Cao harmonizes the other medicinals in this formulaand checks the toxicity <strong>of</strong> Cao Wu and Chuan Wu.ADDITIONS & SUBTRACTIONS: If Lu Xian Cao is difficult to find,it can be substituted by nine grams <strong>of</strong> Wei Ling Xian (RadixClematidis). If Feng Fang is difficult to find, it can be substitutedby three grams <strong>of</strong> Xi Xin (Herba Asari). If pain is movable andnot localized, this is called movable impediment. In that case,add nine grams each <strong>of</strong> Zuan Di Feng (Cortex Schizophragmatis),Qiang Huo (Radix Et Rhizoma Notopterygii),and/or Du Huo (Radix Angelicae Pubescentis). If joint pain issevere with superficial edema, add nine grams <strong>of</strong> Cang Zhu(Rhizoma Atractylodis), 18-21 grams <strong>of</strong> uncooked Yi Yi Ren(Semen Coicis), and 6-9 grams <strong>of</strong> stir-fried Bai Jie Zi (SemenSinapis). If there is a predilection for heat and dread <strong>of</strong> cold andwhen the pain obtains heat it is soothed but, when it obtainscold, it gets worse, this is cold impediment. In that case, one canadd 3-6 grams each <strong>of</strong> Zhi Fu Zi (Radix Lateralis PraeparatusAconiti Carmichaeli), Wu Gong (Scolopendra), and Quan Xie(Scorpio). If joint pain is severe and feels lancinating, this iscalled painful impediment. For this, one can add San Qi (RadixNotoginseng), Mo Yao (Myrrha), Yan Hu Suo (RhizomaCorydalis), Tao Ren (Semen Persicae), Hong Hua (FlosCarthami), Ji Xue Teng (Caulis Spatholobi), and/or other suchmedicinals to quicken the blood and transform stasis. If there isconcomitant spleen qi vacuity, add 15 grams <strong>of</strong> Huang Qi(Radix Astragali) and nine grams <strong>of</strong> Bai Zhu (RhizomaAtractylodis Macrocephalae). If there is concomitant liverdepression, add nine grams each <strong>of</strong> Chai Hu (Radix Bupleuri)and Bai Shao (Radix Paeoniae Albae). For upper limb pain, addnine grams each <strong>of</strong> Qiang Huo (Radix Et Rhizoma Notopterygii)and Sang Zhi (Ramulus Mori). If there is lower limb pain, addnine grams each <strong>of</strong> Niu Xi (Radix Achyranthis Bidentatae) andDu Huo (Radix Angelicae Pubescentis). For severe swelling, addnine grams <strong>of</strong> Ze Xie (Rhizoma Alismatis) and five grams <strong>of</strong> MuTong (Caulis Akebiae). For severe pain, add three grams <strong>of</strong> QuanXie (Scorpio), powdered and taken with the strained decoction.ACUPUNCTURE & MOXIBUSTION: Feng Chi (GB 20), GeShu (Bl 17), Shen Shu (Bl 23), Guan Yuan (CV 4), appropriatelocal pointsANALYSIS OF FORMULA: Draining Feng Chi resolves theexterior and courses wind, while draining Ge Shu quickensthe blood. This combination is based on the statement, “Totreat wind, first treat the blood.” Supplementing Shen Shuand Guan Yuan with moxibustion warms and supplementssource yang so it can scatter cold and warm the channels.Draining the local points frees the flow <strong>of</strong> the network vesselsin the affected area.
