However, qi stagnation and blood stasis may complicateany <strong>of</strong> the preceding patterns.TREATMENT PRINCIPLES: Quicken the blood and movethe qi, transform stasis and rectify the qiRX: Shi Xiao San Jia Wei (Loose a Smile Powder with AddedFlavors)INGREDIENTS:Tao Ren (Semen Persicae)Hong Hua (Flos Carthami)Chi Shao (Radix Paeoniae Rubrae)Dang Gui (Radix Angelicae Sinensis)Pu Huang (Pollen Typhae)Wu Ling Zhi (Feces Trogopterori)Chai Hu (Radix Bupleuri)Zhi Ke (F<strong>ru</strong>ctus Aurantii)Gan Cao (Radix Glycyrrhizae)15g15g15g12g9g9g9g9g6gANALYSIS OF FORMULA: Tao Ren, Hong Hua, Chi Shao, PuHuang, Dang Gui, and Wu Ling Zhi all quicken the bloodand dispel stasis. If there is concomitant bloodvacuity, add 18 grams <strong>of</strong> Ji Xue Teng (Caulis Spatholobi) and12 grams <strong>of</strong> Sheng Di (uncooked Radix Rehmanniae). If thereis concomitant yin vacuity, add 12 grams each <strong>of</strong> He Shou Wu(Radix Polygoni Multiflori) and Gou Qi Zi (F<strong>ru</strong>ctus Lycii).ACUPUNCTURE & MOXIBUSTION: San Yin Jiao (Sp 6), HeGu (LI 4), Tai Chong (Liv 3), Di Cang (St 4), Jia Che (St 6)ANALYSIS OF FORMULA: San Yin Jiao and He Gu togetherquicken the blood and transform stasis throughout the body.Tai Chong and He Gu regulate and rectify the qi <strong>of</strong> the entirebody. In addition, He Gu treats all diseases <strong>of</strong> the head andface. Di Cang and Jia Che free the flow <strong>of</strong> the network vesselslocally. All these points should be drained.ADDITIONS & SUBTRACTIONS: If there is concomitantblood vacuity, add Ge Shu (Bl 17) and Gan Shu (Bl 18). Ifthere is marked spleen vacuity, add Tai Bai (Sp 3) and/or ZuSan Li (St 36).REMARKS1. In real-life practice, most cases <strong>of</strong> oral leukoplakia displaymultipattern presentations. For instance, spleen vacuity andliver depression are common factors in the causation <strong>of</strong>damp heat in addition to hot, spicy and greasy, fatty foodsand alcohol. Since most cases <strong>of</strong> leukoplakia occur in thoseover 60, most patients have qi and yin, qi and yang, or yinand yang vacuities which are then complicated by dampness,heat, qi stagnation, and/or blood stasis. <strong>The</strong>refore, the aboveprotocols will usually have to be combined and modified t<strong>of</strong>it each individual patient.2. Since an element <strong>of</strong> damp heat typically does complicatemany, if not most, cases <strong>of</strong> oral leukoplakia, Chinese dietarytherapy supports the necessity <strong>of</strong> eliminating hot, spicy foodsfrom the diet as well as alcohol. In addition, if dampness isengendered internally due to spleen vacuity, sugars andsweets should also be avoided or minimized.3. If there are damp heat lesions with ulcerations and pain,one can spray the Chinese ready-made medicine Xi GuaShuang (Watermelon Frost) onto the affected area.4. Most oral leukoplakia occurs on the rear edges <strong>of</strong> thetongue where the tongue <strong>ru</strong>bs up against the crowns <strong>of</strong> theteeth. However, unless the tongue is enlarged, it should not<strong>ru</strong>b up against the crowns <strong>of</strong> the teeth. An enlarged tonguesuggests internal accumulation <strong>of</strong> water dampness in turndue to spleen vacuity. <strong>The</strong>refore, we consider spleen vacuityto be an important component <strong>of</strong> this condition and reducingthe enlargement <strong>of</strong> the tongue as an important component<strong>of</strong> its treatment.5. Chewing on the inner surface <strong>of</strong> the cheeks or lips is a type<strong>of</strong> restless stirring or agitation, and, if there is no heat, thereis no agitation. All stirring is an expression <strong>of</strong> yang activity.Commonly such restless stirring is associated with liverdepression transforming heat or yin vacuity with vacuityheat.
