SCLERODERMA 497Pao Jiang (blast-fried Rhizoma Zingiberis) 1.5gMa Huang (Herba Ephedrae) 1.5gANALYSIS OF FORMULA: Shu Di, Shan Zhu Yu, Shan Yao, FuLing, and Dan Pi are five <strong>of</strong> the six ingredients <strong>of</strong> Liu Wei DiHuang Wan (Six Flavors Rehmannia Pills) which enrich kidneyyin to engender kidney yang. Lu Jiao Jiao and Rou Guiwarm kidney yang, while Pao Jiang and Ma Huang warmlung yang. Ji Xue Teng and Yu Jin quicken the blood and freethe flow <strong>of</strong> the network vessels. Bai Jie Zi transforms phlegm,scatters binding or nodulations, and disperses swelling. GanCao harmonizes the other medicinals in this formula.ADDITIONS & SUBTRACTIONS: For severe qi vacuity, add15 grams <strong>of</strong> Huang Qi (Radix Astragali) and 6-9 grams <strong>of</strong>Ren Shen (Radix Ginseng). For more severe cold, add 3-6grams <strong>of</strong> Zhi Fu Zi (Radix Lateralis Praeparatus AconitiCarmichaeli). Moxaing Shen Shu, Zhi Shi, and Ming Men warms andsupplements kidney yang. Draining He Gu and San Yin Jiaomoves the qi and quickens the blood.ADDITIONS & SUBTRACTIONS: If there is concomitantblood vacuity, add Ge Shu (Bl 17) and Gan Shu (Bl 18). Forheart palpitations, add Shen Men (Ht 7) and Xin Shu (Bl 15).For dizziness, moxa Bai Hui (GV 20).REMARKS1. Dampness and phlegm are both yin depressions whichmay hinder and obst<strong>ru</strong>ct the free flow <strong>of</strong> qi and blood, thusdepriving the tissues <strong>of</strong> proper moistening and nourishing.Because qi moves and transforms body fluids, if the qi istoo vacuous to move and transform these properly or if theqi is stagnant and thus not freely flowing, fluids may collectand transform into evil dampness. In addition, becauseblood and body fluids move together, static blood may alsohinder the movement <strong>of</strong> fluids, thus resulting in evil dampness.If evil dampness lingers and endures or if it is congealedby cold or cooked by heat, dampness may transforminto phlegm. Because A) the spleen is the root <strong>of</strong> phlegmengenderment and spleen qi vacuity plays a part in mostpatterns <strong>of</strong> this disease, and B) most patterns <strong>of</strong> this diseasealso include qi stagnation and blood stasis, phlegm anddampness commonly complicate the above patterns <strong>of</strong> scleroderma.When dampness and phlegm complicate the above patterns,one should add the main ingredients <strong>of</strong> Er Chen Tang (TwoAged [Ingredients] Decoction to the above formulas: BanXia (Rhizoma Pinelliae), Fu Ling (Poria), and Chen Pi(Pericarpium Citri Reticulatae). In addition, one may alsoadd Kun Bu (Thallus Algae) and Hai Zao (Sargassum) totransform phlegm and s<strong>of</strong>ten the hard, Tian Nan Xing(Rhizoma Arisaematis) to transform phlegm and free theflow <strong>of</strong> the channels, and Zhi Bai Fu Zi (Rhizoma PraepartaTyphonii) to transform phlegm and dispel wind.2. When scleroderma affects the skin, local treatmentsshould be combined with systemic treatments. <strong>The</strong>se localtreatments should primarily seek to quicken the blood andfree the flow <strong>of</strong> the network vessels. For instance, one canseven star hammer each day all around the margins <strong>of</strong> anysclerotic areas. If stasis is complicated by cold, one can soakthe affected areas, especially if they are located on the fingers,in warm decoctions <strong>of</strong> acrid and warm, quickening andmoving medicinals, such as Xi Xin (Herba Asari), Gui Zhi(Ramulus Cin<strong>nam</strong>omi), Su Mu (Lignum Sappan), Xue Jie(Sanguis Draconis), etc. A warm, moving tincture for externalapplication can be made from 60 grams each <strong>of</strong> DangGui (Radix Angelicae Sinensis) and Rou Gui (CortexCin<strong>nam</strong>omi), 30 grams each <strong>of</strong> Hong Hua (Flos Carthami),Gan Jiang (dry Rhizoma Zingiberis), and Chuan Jiao(F<strong>ru</strong>ctus Zanthoxyli), and 15 grams each <strong>of</strong> Zang Nao(Camphora) and Xi Xin (Herba Asari). Soak these in oneliter <strong>of</strong> 95% alcohol for one week. <strong>The</strong>n strain the dregs andbottle the resulting tincture. Apply several times per day.Combining massage <strong>of</strong> the affected area with this externalapplication increases its effect.3. Er Wu Tong Bi Tang (Two Wu’s Free the Flow <strong>of</strong>Impediment Decoction) achieves good results for enduringscleroderma. It works especially well when skin abnormalities(pi bi, skin impediment) are accompanied by emaciationor muscular atrophy and restricted movement (i.e., jin bi,sinew impediment), painful joints and bones (gu bi, boneimpediment), and lesions <strong>of</strong> the internal organs (wu zang bi,five viscera impediment). Its ingredients are: uncookedHuang Qi (Radix Astragali), Shen Jin Cao (Herba Lycopodii),and Lian Qiao (F<strong>ru</strong>ctus Forsythiae), 12g each, processedChuan Wu (Radix Aconiti Carmichaeli), processed Cao WuTou (Radix Aconiti Kusnez<strong>of</strong>ii), Gui Zhi (RamulusCin<strong>nam</strong>omi), Han Fang Ji (Radix Stephaniae), Dang Gui(Radix Angelicae Sinensis), Sang Ji Sheng (Herba Taxilli),Chuan Niu Xi (Radix Cyathulae), and Xuan Shen (RadixScrophulariae), 9g each, Qin Jiao (Radix GentianaeMacrophyllae) and Fang Feng (Radix Saposhnikoviae), 6geach, Qiang Huo (Radix Et Rhizoma Notopterygii) and Du
