73STRESS INCONTINENCEStress incontinence refers to the involuntary loss <strong>of</strong> urine oncoughing, straining, sneezing, lifting, or any maneuver thatsuddenly increases intra-abdominal pressure. While this conditionmay sometimes be seen in men after prostatectomy, itis the most common cause <strong>of</strong> involuntary loss <strong>of</strong> urine inwomen. It may be due to shortening <strong>of</strong> the urethra and loss<strong>of</strong> normal posterior urethrovesical angle resulting from pelvicrelaxation secondary to aging or multiparity.<strong>The</strong> <strong>Western</strong> medical diagnosis <strong>of</strong> this condition is based inthe patient’s history, pelvic examination, and demonstratingloss <strong>of</strong> urine with coughing or straining, which may bestopped by finger elevation <strong>of</strong> the paraurethral vaginal tissuesat the neck <strong>of</strong> the bladder. Mild cases <strong>of</strong> this condition mayrespond to pubococcygeal muscle exercises (i.e., Kegels) or tosympathomimetic d<strong>ru</strong>g therapy. Severe cases may requiresurgery.CHINESE DISEASE CATEGORIZATION: Urinaryincontinence is called niao shi jin, xiao bian bu jin, and xiaobian shi jin in Chinese.DISEASE CAUSES: Former heaven natural endowmentinsufficiency, aging, enduring diseases, multiparity, internaldamage by the seven affects, and unregulated eating anddrinkingDISEASE MECHANISMS: Because this condition is mostcommonly seen in the same age women as interstitial cystitisand both conditions have to do with abnormal discharge <strong>of</strong>urine, their disease causes and mechanisms are essentially thesame. Although textbooks divide incontinence in generalinto lung-spleen qi vacuity, lower source vacuity cold (i.e.,kidney yang vacuity), and liver channel depressive fire types,according to most Chinese authorities, a spleen-kidney dualvacuity lies at the center <strong>of</strong> the overwhelming majority <strong>of</strong>cases <strong>of</strong> stress incontinence.
