San Yin Jiao nourishes the blood, while Zu San Li supplementsthe spleen and boosts the qi.ADDITIONS & SUBTRACTIONS: For cyanosis <strong>of</strong> the fingertipsand toes, add Ba Xie (M-UE-22) or Ba Feng (M-LE-8). Ifthere is liver depression qi stagnation, add Tai Chong (Liv 3).REMARKS1. As with most other autoimmune diseases, polymyositis/dermatomyositis is most commonly found in females, andfemales, due to menst<strong>ru</strong>ation, gestation, and lactation, aremore apt to exhibit spleen vacuity than men. Such spleen vacuitytends to become more pronounced at around 35 years <strong>of</strong> agein women. Because the spleen becomes vacuous and weak and,therefore, does not engender and transform blood as abundantlyas before, the liver tends to become or become moredepressed. If spleen qi vacuity reaches the kidneys, spleen qivacuity will evolve into spleen-kidney yang vacuity. If liverblood vacuity reaches the kidneys, liver blood vacuity willevolve into kidney yin vacuity. As it is said, “Yin is automaticallyhalf at 40 years <strong>of</strong> age.” Because yin and yang are mutuallyrooted, vacuity <strong>of</strong> one will commonly involve at least someelement or lead to vacuity <strong>of</strong> the other. Thus, in real-life clinicalpractice, it is not uncommon to see <strong>Western</strong> women in their40s with autoimmune diseases who display liver depression qistagnation and spleen qi, liver blood, and kidney yin and yangvacuities. Because <strong>of</strong> the interrelationships between the qi,blood, and fluids and humors, these five patterns may also becomplicated by blood stasis, damp accumulation, phlegmobst<strong>ru</strong>ction, and/or some kind <strong>of</strong> evil heat. In such complicated,multi-pattern scenarios, which are the <strong>ru</strong>le rather than theexception, one must treat all <strong>of</strong> these disease mechanisms at thesame time since they are all mutually engendering.2. Whenever enduring disease has caused blood stasis in thenetwork vessels, one must use medicinals which quicken theblood and free the flow <strong>of</strong> the network vessels. Such medicinalsgenerally belong to one <strong>of</strong> three categories: 1) worm and insectmedicinals, e.g., Quan Xie (Scorpio), Wu Gong (Scolopendra),and Wu Shao She (Zaocys), 2) tree saps and resins, e.g., RuXiang (Olibanum), Mo Yao (Myrrha), and Xue Jie (SanguisDraconis), and 3) medicinals which are network-like in form,e.g., Si Gua Luo (Fasciculus Luffae) and Ju Luo (Fasciculus CitriReticulatae). When dealing with rheumatic complaints, theworm and insect medicinals are usually the most effective <strong>of</strong>these three categories.3. During the initial stage <strong>of</strong> skin lesions, one can make a washout <strong>of</strong> 30 grams <strong>of</strong> Tou Gu Cao (Herba Impatientis), 25 grams<strong>of</strong> Gui Zhi (Ramulus Cin<strong>nam</strong>omi), and nine grams <strong>of</strong> HongHua (Flos Carthami). Decoct these three medicinals in waterand use the resulting medicinal liquid as an external washapplied warm one time per day.4. Er Shen Er Teng Tang (Two Roots & Two Vines Decoction)treats dermatomyositis presenting as qi, blood, and yin vacuitywith blood stasis and heat toxins: Ji Xue Teng (CaulisSpatholobi) and Luo Shi Teng (Caulis Trachelospermi), 30geach, Huang Qi (Radix Astragali), 20g, Dang Shen (RadixCodonopsitis), Bei Sha Shen (Radix Glehniae), and Sheng Di(uncooked Radix Rehmanniae), 15g each, and Dan Pi(Cortex Moutan) and Zi Cao (Radix Arnebiae/Lithospermi),12g each. During the initial stage with fever, throat pain, redmacules, and red tongue, add 12 grams each <strong>of</strong> Da Qing Ye(Folium Daqingye) and Jin Yin Hua (Flos Lonicerae) and 18-30 grams <strong>of</strong> Pu Gong Ying (Herba Taraxaci). For muscle painand aversion to cold due to concomitant yang vacuity withwind damp evils, add nine grams each <strong>of</strong> Zhi Fu Zi (RadixLateralis Praeparatus Aconiti Carmichaeli), Yin Yang Huo(Herba Epimedii), and Qiang Huo (Radix Et RhizomaNotopterygii ). For enduring disease leading to blood stasis,add nine grams each <strong>of</strong> Dan Shen (Radix SalviaeMiltiorrhizae ) and Hong Hua (Flos Carthami).
