226 THE TREATMENT OF MODERN WESTERN DISEASES WITH CHINESE MEDICINE<strong>The</strong> <strong>Western</strong> medical treatment <strong>of</strong> diabetes primarily relies ondietary avoidance <strong>of</strong> sugars and sweets, the regular scheduling<strong>of</strong> meals, weight loss (for NIDDM), and, when necessary,insulin replacement therapy. <strong>The</strong>re are as many as seven differenttypes <strong>of</strong> injectible insulin currently prescribed in the UnitedStates, each having their own time to onset <strong>of</strong> action, peakaction, and duration <strong>of</strong> action. Complications <strong>of</strong> insulin treatmentinclude insulin shock, i.e., hypoglycemia, if too muchinsulin or too little food are taken, local reactions to insulininjections, such as heat, induration, erythema, and urticaria,and insulin resistance. Several oral sulfonylureates that lowerblood glucose level may be used to treat selected patients.However, these are not adequate for treating IDDM patients.When hypertension complicates diabetes, diuretics and sympatheticinhibitors may accelerate or worsen impotence.CHINESE DISEASE CATEGORIZATION: Diabetes mellitusis traditionally categorized as xiao ke, wasting thirst or wastingand thirsting. However, because polyphagia, polydipsia, andpolyuria are three <strong>of</strong> the most common symptoms <strong>of</strong> this condition,it also falls under duo shi, pr<strong>of</strong>use eating, duo yin, pr<strong>of</strong>usedrinking, and duo niao, pr<strong>of</strong>use urination. Other conditionsassociated with this disease include fei pang, obesity, yangwei, impotence, ma mu, tingling and numbness, chuang yang,sores, and qing mang, clear-eyed blindness.DISEASE CAUSES: Former heaven natural endowmentinsufficiency, aging, unregulated eating and drinking, andinternal damage by the seven affectsDISEASE MECHANISMS: In juvenile-onset diabetes, the diseasemechanism appears to be a natural endowment insufficiency,whereas, in adults, this disease typically begins withlong-standing heat in the stomach coupled with spleen vacuity.4 This heat may be due to over-eating hot, spicy, greasy,fatty, thick-flavored foods or alcohol or to depressive heat <strong>of</strong>the liver and stomach and gives rise to a large appetite and/orrapid hungering after meals. <strong>The</strong> spleen vacuity may be due toover-eating sugars and sweets and/or fatty, thick-flavoredfoods, too much thinking, too little exercise, and too muchtaxation fatigue. Spleen vacuity is responsible for obesity andfatigue initially and also for anorexia, emaciation, and muscularatrophy as this disease progresses. If heat endures, it eventuallydamages and consumes yin fluids in the stomach andlungs, giving rise to polydipsia. If lung-stomach yin vacuityreaches the kidneys and is complicated by yin vacuity due toaging, lung-stomach yin vacuity may evolve into kidney yinvacuity. If spleen qi vacuity evolves into kidney yang vacuity,there may be kidney yin and yang dual vacuity. Kidney yinand/or yang vacuity give rise to urinary problems as well asimpotence. Because all adult diabetics also exhibit signs andsymptoms <strong>of</strong> liver depression qi stagnation, long-term qi stagnationcoupled with qi and blood vacuity, typically gives riseto blood stasis. In addition, spleen vacuity <strong>of</strong>ten also becomescomplicated by damp heat which pours downward, resultingin sores, urinary disturbances, impotence, restless leg syndrome,and/or vaginitis.TREATMENT BASED ON PATTERNDISCRIMINATION:1. Xuan Shen clears vacuity heat. BaiShao nourishes the blood, remembering that blood and fluidsshare a common source. It also restrains the liver, rememberingthat the liver and lungs work hand in hand to control theflow <strong>of</strong> qi, and if one becomes vacuous and weak, the othertends to become exuberant and effulgent. Dan Pi quickensand clears heat from the blood. In this formula, it helps preventflaring <strong>of</strong> ministerial fire due to mutual engenderment.Gan Cao harmonizes the other medicinals. Mai Men Dong,Tian Men Dong, Zhi Mu, Sheng Di, Tian Hua Fen, and XuanShen have all also been shown to lower blood sugar.ADDITIONS & SUBTRACTIONS: If there is liver depression
