110 THE TREATMENT OF MODERN WESTERN DISEASES WITH CHINESE MEDICINEREMARKS1. When treating CTS with acupuncture, it is usual to needledirectly into the carpal tunnel at Da Ling (Per 7). Usestrong stimulation and do not retain the needle. In somecases, needling Nei Guan (Per 6) and Lao Gong (Per 8) mayachieve an even better effect.2. Most patients with CTS also have liver depression qi stagnation,if not before, then as a sequela to the f<strong>ru</strong>stration <strong>of</strong>not being able to work or play. Depending on how prominentthis liver depression is, one may choose to modify suchformulas as Xiao Chai Hu Tang (Minor Bupleu<strong>ru</strong>mDecoction) or Xiao Yao San (Rambling Powder) with bloodnourishing,blood-quickening, and/or wind damp treatingmedicinals. In other words, for best results, take the bodilyconstitution as the root and modify a guiding formula forthat constitution for the disease mechanisms specificallyassociated with CTS.3. Cessation <strong>of</strong> activities aggravating CTS, at least longenough for the condition to heal, are a prerequisite for itssuccessful Chinese medical treatment. Often recurrences <strong>of</strong>this condition can be prevented either by ergonomicallymore efficient tools and appliances or kinesiological re-educationand counseling. <strong>The</strong>refore, patients with work orsports-related CTS should be referred to a physiatrist, occupationaltherapist, kinesiologist, etc.4. External applications <strong>of</strong> Chinese herbal liniments, ointments,or fomentations may help speed recovery. In thatcase, one should use a liniment, ointment, or fomentationwhose main medicinals match the patient’s pattern. Suchexternal treatments are more effective for qi stagnation andblood stasis and wind damp impediment patterns <strong>of</strong> CTSand are not as effective for blood vacuity not nourishing thesinews and vessels. Heat therapy with a TDP lamp may helpall patterns <strong>of</strong> CTS.
