29FANCONI’S SYNDROMEFanconi’s syndrome is an inherited or acquired disorder involvingabnormalities <strong>of</strong> renal proximal tubular function. <strong>The</strong>seabnormalities include glucosuria, phosphaturia, aminoaciduria,and bicarbonate wasting. When inherited, Fanconi’ssyndrome usually accompanies other genetic disorders, such ascystinosis, Wilson’s disease, hereditary f<strong>ru</strong>ctose intolerance,Lowe’s syndrome, tyrosinemia, galactosemia, and glycogenstorage disease. Acquired Fanconi’s syndrome may be due toheavy metal intoxication or other chemical agents, renal transplantation,multiple myeloma, or vitamin D deficiency. Ininherited Fanconi’s syndrome, the main clinical features arepolyuria, polydispsia, and rickets which usually appear in infancy.In acquired Fanconi’s syndrome, adult patients commonlypresent with osteomalacia and muscle weakness. <strong>Western</strong> medicaldiagnosis <strong>of</strong> this condition is mostly based on blood analysisdemonstrating abnormal renal function. <strong>The</strong>re is no specific<strong>Western</strong> treatment <strong>of</strong> this syndrome. If caused by a nephrotoxin,removal <strong>of</strong> that toxin from the patient’s environment isessential.CHINESE DISEASE CATEGORIZATION: Based on its clinicalmanifestation, this disease falls under the categories <strong>of</strong>turbid urination (niao zhuo) and yao tong (low back pain) inChinese medicine.DISEASE CAUSES: This condition is mostly due to formerheaven natural endowment insufficiency and extreme taxationand fatigue causing detriment and damage to the kidneyorigin. It may also be due to overeating sweet, fatty foods andan unregulated diet which damages the spleen qi and causesloss <strong>of</strong> nourishment <strong>of</strong> the kidneys.DISEASE MECHANISMS: <strong>The</strong> kidneys <strong>ru</strong>le the bones, and thelow back is the mansion <strong>of</strong> the kidneys. <strong>The</strong>refore, bone pain ingeneral and low back pain in particular are related to the kidneys.If the spleen becomes vacuous and the central qi fallsdownward or if kidney vacuity results in securing and gatheringlosing their duty, the finest essence many flow downward resultingin turbid urination.
