For tremors <strong>of</strong> the hands or fingers, add He Gu (LI 4) andTai Chong (Liv 3). For a sensation <strong>of</strong> throat obst<strong>ru</strong>ction, addLiang Quan (CV 23) and Tong Li (Ht 5). For pr<strong>of</strong>use sweating,add He Gu (LI 4) and Fu Liu (Ki 7).REMARKS1. <strong>The</strong> disease mechanisms <strong>of</strong> this condition are a combination<strong>of</strong> vacuity and repletion. <strong>The</strong> root vacuities in this conditionare mainly yin vacuity, qi vacuity, and, if severe, yangvacuity. <strong>The</strong> branch repletions are qi stagnation, phlegmcongelation, and blood stasis.2. During the initial stage <strong>of</strong> this disease, liver depression andphlegm nodulation is the most common pattern. <strong>The</strong>refore,one should mainly out-th<strong>ru</strong>st depressed wood. During themiddle stage, the emphasis is usually on clearing the liver anddraining heat. At this stage, the disease is mostly in the liver,heart, and stomach. During the latter stage, the emphasis ismostly on supplementing qi and yin vacuities. At this stage,the disease is primarily in the liver, heart, spleen, and kidneys.3. Medicinals which scatter phlegm nodulation in the region<strong>of</strong> the throat include Xia Ku Cao (Spica P<strong>ru</strong>nellae), XuanShen (Radix Scrophulariae), and Huang Yao Zi (RhizomaDioscoreae Bulbiferae). P<strong>ru</strong>nella tends to be used more forrepletion patterns, while Dioscoreae Bulberifera is used morefor vacuity patterns and especially qi vacuity. Scrophulariaecan be used for either repletion or vacuity. Medicinals whichtransform phlegm and scatter nodulation include Ban Xia(Rhizoma Pinelliae), Bai Jie Zi (Semen Sinapis), and Zhe BeiMu (Bulbus Fritillariae Thunbergii). And medicinals whichs<strong>of</strong>ten the hard and scatter nodulation include Long Gu (OsDraconis), Mu Li (Concha Cyclinae Ostreae), Hai Ge Ke(Concha Meretricis/Cyclinae), Hai Fu Shi (Pumice), Kun Bu(Thallus Algae), Hai Zao (Sargassum), and Hai Dai (ThallusLaminariae).4. When Huang Qi (Radix Astragali) is used in heavy doses(25-50g), it can lower T3 & T4, and effectively treats hyperthyroidism.Huang Yao Zi (Rhizoma DioscoreaeBulberiferae) is another important medicinal for hyperthyroidism.However, it is hepatotoxic. <strong>The</strong>refore, its doseshould not exceed 10 grams and should only be used for alimited period <strong>of</strong> time.5. Because hyperthyroidism is basically a yin-yang imbalancewith yang surplus causing heat and stirring and yin vacuityresulting in lack <strong>of</strong> stillness and quiet, it is important to alsomodify the diet and lifestyle in cases <strong>of</strong> hyperthyroidism. Interms <strong>of</strong> lifestyle, stress and taxation should both be minimized.In terms <strong>of</strong> diet, one should avoid acrid, hot, drying,and stimulating foods, such as hot spices, alcohol, c<strong>of</strong>fee andstrong tea, etc. On the other hand, if there are spleen vacuityand phlegm, one should avoid uncooked, chilled foods aswell as oils and fats, including those in dairy products suchas milk and cheese.6. <strong>The</strong> following modern formulas all treat complex multipatternpresentations:Ping Ying Fu Fang (Level Goiter Compound Formula) treatsa yin blood vacuity with qi and blood stagnation and phlegmnodulation. It consists <strong>of</strong>: Mu Li (Concha Ostreae), 20g, XiaKu Cao (Spica P<strong>ru</strong>nellae), 15g, Wa Leng Zi (Concha Arcae),Xuan Shen (Radix Scrophulariae), Bai Shao (Radix PaeoniaeAlbae), Dan Pi (Cortex Moutan), Sheng Di (uncooked RadixRehmanniae), Dang Gui (Radix Angelicae Sinensis), and FuLing (Poria), 12g each, and Shan Zhu Yu (F<strong>ru</strong>ctus Corni),Zhe Bei Mu (Bulbus Fritillariae Thunbergii), Qing Pi(Pericarpium Citri Reticulatae Viride), Chen Pi (PericarpiumCitri Reticulatae), San Leng (Rhizoma Sparganii), and E Zhu(Rhizoma Curcumae), 9g each.Jia Kang Ping (Hyperthyroidism Balancer) treats qi and yinvacuity with phlegm nodulation. It consists <strong>of</strong>: Tai Zi Shen(Radix Pseustellariae) and Mu Li (Concha Ostreae), 30geach, Sheng Di (uncooked Radix Rehmanniae) and Hai GeKe (Concha Meretricis/Cyclinae), 15g each, Shi Hu (HerbaDendrobii), Zhe Bei Mu (Bulbus Fritillariae Thunbergii),and Xia Ku Cao (Spica P<strong>ru</strong>nellae), 12g each, and Mai MenDong (Tuber Ophiopogonis) and Xuan Shen (RadixScrophulariae), 9g each.Jia Kang Jian Tang (Hyperthyroidism Decoction) treats qiand yin vacuity with liver depression transforming heat andphlegm nodulation. It consists <strong>of</strong>: Bai Shao (Radix PaeoniaeAlbae), Wu Mei (F<strong>ru</strong>ctus Mume), Mu Gua (F<strong>ru</strong>ctusChaenomelis), Bei Sha Shen (Radix Glehniae), Mai MenDong (Tuber Ophiopogonis), Shi Hu (Herba Dendrobii),Bai Bian Dou (Semen Dolichoris), and Lian Zi (SemenNelumbinis), 12g each, Kun Bu (Thallus Algae) and Hai Zao(Sargassum), 9g each, and Chai Hu (Radix Bupleuri), Sang Ye(Folium Mori), and Zhi Zi (F<strong>ru</strong>ctus Gardeniae), 6g each.
