Foredema, increase the dosage <strong>of</strong> Ze Xie and Fu Ling up to 15 gramsand add 12 grams each <strong>of</strong> Wu Jia Pi (Cortex Acanthopanacis)and Bai Zhu (Rhizoma Atractylodis Macrocephalae). Formarked low back and knee pain, add 12 grams each <strong>of</strong> Sang JiSheng (Herba Taxilli), Niu Xi (Radix Achyranthis Bidentatae),and Xu Duan (Radix Dipsaci). For spine and low back painaccompanied by a cold sensation, add 12 grams each <strong>of</strong> Gou Ji(Rhizoma Cibotii), Xu Duan (Radix Dipsaci), and Du Zhong(Cortex Eucommiae). For heart palpitations, replace Rou Guiwith Gui Zhi (Ramulus Cin<strong>nam</strong>omi) and add 12 grams each <strong>of</strong>Dan Shen (Radix Salviae Miltiorrhizae) and Chuan Xiong(Rhizoma Chuanxiong). For abdominal distention, add ninegrams <strong>of</strong> Ban Xia (Rhizoma Pinelliae) and six grams <strong>of</strong> Chen Pi(Pericarpium Citri Reticulatae). For poor appetite, add ninegrams each <strong>of</strong> Shen Qu (Massa Medica Fermentata), Shan Zha(F<strong>ru</strong>ctus Crataegi), and Mai Ya (F<strong>ru</strong>ctus Germinatus Hordei)and six grams <strong>of</strong> Sha Ren (F<strong>ru</strong>ctus Amomi). If there is phlegmnodulation (i.e., goiter), add 15 grams each <strong>of</strong> Xiao Ku Cao(Spica P<strong>ru</strong>nellae), Xuan Shen (Radix Scrophulariae), and Mu Li(Concha Ostreae), 12 grams <strong>of</strong> Zhe Bei Mu (Bulbus FritillariaeThunbergii), and nine grams each <strong>of</strong> Hai Zao (Sargassum), andKun Bu (Thallus Algae). If there is simultaneous blood stasis,add 15 grams <strong>of</strong> Dan Shen (Radix Salviae Miltiorrhizae). If thereis wind damp cold impediment, add 15 grams each <strong>of</strong> Wu JiaPi (Cortex Acanthopanacis) and Ji Xue Teng (Caulis Spatholobi).If there is depressive heat, add 9-12 grams <strong>of</strong> Huang Qin (RadixScutellariae). If there is concomitant yin vacuity with easy fright,insomnia, night sweats, dizziness, hot flashes in the face, dryskin with possible desquamation, dry, lusterless hair, and constipation,add 12 grams each <strong>of</strong> Nu Zhen Zi (F<strong>ru</strong>ctus LigustriLucidi), Wu Wei Zi (F<strong>ru</strong>ctus Schisandrae), and Han Lian Cao(Herba Ecliptae) and increase the dosage <strong>of</strong> Shu Di up to 15-20g.If there is a marked heart yang vacuity coupled with kidneyyang vacuity, a combination <strong>of</strong>ten seen in hypothyroidism,accompanied by cardiac effusion, heart palpitations, an emptysensation in the heart, shortness <strong>of</strong> breath, possible chestoppression or pain, aversion to cold, cold limbs, a pale andpuffy face, apathy and mental slowness, fatigue, dizziness, tinnitus,weak limbs, hypersomnia, a pale tongue with thin, whitefur, and a deep, slow, weak, possibly bound or regularly inter<strong>ru</strong>ptedpulse, replace Jia Wei Shen Qi Wan with Zhi Gan CaoTang Jia Jian (Mix-fried Licorice Decoction with Additions &Subtractions): Huang Qi (Radix Astragali), 30g, Sheng Di(uncooked Radix Rehmanniae), 20g, mix-fried Gan Cao(Radix Glycyrrhizae), 15g, and Zhi Fu Zi (Radix LateralisPraeparatus Aconiti Carmichaeli), Dang Shen (RadixCodonopsitis Pilosulae), Dang Gui (Radix Angelicae Sinensis),Chuan Xiong (Rhizoma Chuanxiong), Gui Zhi (RamulusCin<strong>nam</strong>omi Cassiae), Bai Shao (Radix Paeoniae Albae), MaiMen Dong (Tuber Ophiopogonis), and Wu Wei Zi (F<strong>ru</strong>ctusSchisandrae), 9g each. If the pulse is weak, slow, and deep, addsix grams <strong>of</strong> Ma Huang (Herba Ephedrae) and three grams <strong>of</strong>Xi Xin (Herba Asari). If the pulse is faint, bound, or regularlyinter<strong>ru</strong>pted, add six grams <strong>of</strong> Zhi Shi (F<strong>ru</strong>ctus Immatu<strong>ru</strong>sAurantii) and replace Dang Shen with six grams <strong>of</strong> Ren Shen(Radix Ginseng). If there is chest oppression and pain, add 24grams <strong>of</strong> Gua Lou Pi (Pericarpium Trichosanthis) and ninegrams each <strong>of</strong> Xie Bai (Bulbus Allii Macrostemi), Yan Hu Suo(Rhizoma Corydalis), and Dan Shen (Radix SalviaeMiltiorrhizae).ACUPUNCTURE & MOXIBUSTION: Pi Shu (Bl 20), ShenShu (Bl 23), Ming Men (GV 4), Guan Yuan (CV 4)ANALYSIS OF FORMULA: With moxibustion and supplementingmethod, Pi Shu and Guan Yuan supplement the latterheaven, while Shen Shu and Ming Men supplement theformer heaven. Together, they fortify the spleen and boostthe qi, supplement the kidneys and invigorate yang.NOTE: This seemingly simple treatment achieves goodresult, but needs a long course (e.g., 3-6 months).Moxibustion should use nonscarring direct thread moxa.Warming these points with a moxa roll is not adequate.ADDITIONS & SUBTRACTIONS: If there is facial edema,add Yin Ling Quan (Sp 9) and Shui Gou (GV 26). If there isgeneralized edema, add Yin Ling Quan (Sp 9), Shui Fen (CV9), and San Yin Jiao (Sp 6). If there is constipation, add TianShu (St 25).
