For severe jaundice,increase the dosage <strong>of</strong> Bai Zhu up to 15 grams and that <strong>of</strong>Yin Chen Hao up to 25 grams.ACUPUNCTURE & MOXIBUSTION: Zu San Li (St 36),Zhong Wan (CV 12), Yin Ling Quan (Sp 9), Tai Chong (Liv3), Zhang Men (Liv 13)ANALYSIS OF FORMULA: Zhong Wan, Yin Ling Quan, andZu San Li fortify the spleen, warm yang, and transformdampness. Zhang Men rectifies the qi and disperses accumulationsin the rib-side. Tai Chong recedes jaundice. All thesepoints should be drained except Zu San Li. Also moxa ZuSan Li and Zhong Wan.ADDITIONS & SUBTRACTIONS: For marked fatigue and lack <strong>of</strong>strength, add Tai Bai (Sp 3). For severe jaundice, add Gan Shu(Bl 18) and Pi Shu (Bl 20). For upper abdominal distention,add Shang Wan (CV 13). For abdominal distention at the level<strong>of</strong> the waist, add Tian Shu (St 25). For nausea and vomiting,add Nei Guan (Per 6). For diarrhea, add Shen Que (CV 8).REMARKS1. Ultimately, no matter what the disease, one should alwaystreat the patient’s personal pattern or combination <strong>of</strong> patterns.Although <strong>Western</strong> MDs say that chronic active hepatitismay be asymptomatic, patients with this condition arenever asymptomatic from the point <strong>of</strong> view <strong>of</strong> Chinese medicine.In other words, every person with chronic active hepatitisdoes display one or more patterns, remembering that aChinese pattern is made up <strong>of</strong> a combination <strong>of</strong> signs andsymptoms, tongue signs, and pulse signs. <strong>The</strong>se signs andsymptoms may not have anything to do with hepatitis froma <strong>Western</strong> medical point <strong>of</strong> view. For instance, women withliver depression qi stagnation, typically have menst<strong>ru</strong>al andpremenst<strong>ru</strong>al symptoms. Men will at least be irritable or easilyf<strong>ru</strong>strated. <strong>The</strong>refore, it is extremely important to do acomplete intake using the four examinations. Even if there isnothing other than an enlarged tongue with a red right edgeand slimy tongue fur plus a bowstring, slippery pulse, thisdoes add up to a Chinese medical pattern.At the very least, there is usually a liver-stomach and/or liverspleendisharmony. <strong>The</strong> above are the most commonly seenpatterns in chronic active hepatitis. However, they merelygive some general guidelines to pattern discrimination andtreatment. Individual treatment plans will usually be a combination<strong>of</strong> several <strong>of</strong> the above elements.2. It has recently been reported from Japan that severalpatients on interferon therapy have died due to liver failurewhile simultaneously taking Xiao Chai Hu Tang (MinorBupleu<strong>ru</strong>m Decoction). As <strong>of</strong> this writing, the exact pharmacody<strong>nam</strong>icshave not been determined. Some synergismbetween interferon and Chai Hu (Radix Bupleuri) is suspected.However, it may also be that Xiao Chai Hu Tang was misprescribedto these patients. Until this issue has been clearedup, we advise not prescribing Bupleu<strong>ru</strong>m with interferon.3. Although a combination <strong>of</strong> enduring damp heat evils andaging <strong>of</strong>ten damage and consume the spleen qi resulting inspleen vacuity with cold dampness and even spleen-kidneyyang vacuity, care should be taken when using interior-
