4 THE TREATMENT OF MODERN WESTERN DISEASES WITH CHINESE MEDICINEone says that a patient’s pattern was liver depression qi stagnationwith transformative heat, one is using three different systems<strong>of</strong> pattern discrimination. By saying liver and not heartor stomach, one is using viscera and bowel pattern discrimination.Saying that there is transformative heat is a type <strong>of</strong> eightprinciples pattern discrimination since the discrimination <strong>of</strong>heat from cold is part <strong>of</strong> that system. In addition, liver depressionis a species <strong>of</strong> repletion, and the discrimination <strong>of</strong> repletionfrom vacuity is another part <strong>of</strong> eight principles patterndiscrimination. As another example, if one says that thepatient’s pattern is liver wood assailing and damaging thespleen, one has specifically used viscera bowel pattern discriminationand five phases pattern discrimination. One has alsoimplied eight principles pattern discrimination, since liverassailing spleen means that, due to the liver being replete, thespleen has become vacuous.Of these 10 systems, only the last three tend to be used asstand-alone systems, and even the last two are typically combined.System No. 8 refers to the six divisions <strong>of</strong> cold damageoriginated by Zhang Zhong-jing. Of these six divisions,only the shao yang division is still regularly used in <strong>Western</strong>clinical practice. 5 Systems No. 9 and 10 are both parts <strong>of</strong>Qing dynasty warm disease theory. <strong>The</strong> four divisions are thedefensive, qi, const<strong>ru</strong>ctive, and blood. <strong>The</strong>se describe thecommon routes <strong>of</strong> transmission <strong>of</strong> externally contractedwarm heat evils in the body. Three burner pattern discrimination<strong>of</strong>fers yet further elaboration on the routes <strong>of</strong> transmission<strong>of</strong> such warm heat evils through the three burners.THE THREE CONSTITUENTS OF ACHINESE MEDICAL PATTERNChinese medical patterns are made up <strong>of</strong> three groups orstreams <strong>of</strong> information gathered by the four examinations.Those three groups <strong>of</strong> information are 1) general signs andsymptoms, 2) tongue signs, and 3) pulse signs. First <strong>of</strong> all,only information gathered by the four examinations is germanewith making a Chinese pattern discrimination.Although research is currently being conducted in thePeople’s Republic <strong>of</strong> China trying to ascertain what <strong>Western</strong>medical laboratory tests reliably correspond to Chinese patterns6 , no such test is yet considered standard in pr<strong>of</strong>essionalChinese medicine. Secondly, it is the coordination <strong>of</strong> signsand symptoms, tongue signs, and pulse signs which make upa Chinese medical pattern. No single sign or symptom equalsa pattern. By its very nature, a pattern must be made up <strong>of</strong>more than a single element. 7 <strong>The</strong> meaning <strong>of</strong> any single signor symptom only becomes apparent in the light <strong>of</strong> the othersigns and symptoms with which it appears. If any element <strong>of</strong>these three groups <strong>of</strong> information is materially different, thenone is dealing with a different pattern. If all three groups <strong>of</strong>information are the same, then the patterns are the same, andtheir different <strong>nam</strong>es are only that, different <strong>nam</strong>es for essentiallythe same diagnostic entity.THE STEPS IN THE CHINESE MEDICALPRESCRIPTIVE METHODOLOGYPr<strong>of</strong>essional Chinese medicine is a rational medicine, andthere is a definite step-by-step process taught at Chinesemedical schools for arriving at correct treatment plans. 8 <strong>The</strong>table below outlines the steps in this logical process.This step-by-step methodology is the standard <strong>of</strong> care in pr<strong>of</strong>essionallypracticed Chinese medicine in the People’sRepublic <strong>of</strong> China, and it is only by reliance on such a codifiedprescriptive methodology that one can have a nationalTHE STEPS IN THE CHINESE MEDICAL PRESCRIPTIVE METHODOLOGY1. Gather information via the four examinations2. Analyze that data according to Chinese medicaltheory3. Determine the modern <strong>Western</strong> disease diagnosis (orwrite down such a pre-established diagnosis)4. Determine the traditional Chinese medical diseasediagnoses5. Determine the Chinese pattern discrimination6. State the treatment principles necessary to rebalance theimbalance implied in the <strong>nam</strong>e(s) <strong>of</strong> the patient’s pattern(s)7. Choose treatment methods logically derived fromthose treatment principles8. Administer that treatment9. Assess the outcome <strong>of</strong> that treatment10. Reframe the patients’ pattern discrimination based onthat assessment11. Reframe the treatment principles based on the newpattern discrimination12. Modify the treatment plan to embody these new orrevised treatment principles13. Continue with steps 9-13 until the patient is cured orstabilized
